Employee did nt feel well earlier in the day. Went up to catwalk to repair weld near screen deck. While walking on screen deck, became dizzy & possibly lost conciousness and fell hitting his head. Required stitches & overnight observation at hospital.

HE WAS ASSISTING IN REMOVING A CONVEYOR HOPPER & CAUGHT HIS THUMB CAUSING A CUT & FRACTURE TO THE UPPER RIGHT THUMB.

DURING THE PROCESS OF TURNING CONE DOWN, WHICH INVOLVES JACKING, RT HAND & 2 FINGERS WERE PINCHED LETTING JACK DOWN. CONE WAS NOT COMPLETELY DOWN & LANDED ON HAND.
