EE WAS TRAMMING IN REVERSE AND WATCHING THE COMPRESSOR WHEN EE STEPPED ON A LOOSE STONE THROWING HIM OFF BALANCE. HE FELL BACKWARDS & FAILED TO RELEASE THE TRAMMING CONTROLS ON THE AIR TRACK, CAUSING THE DRILL TO BACK OVER THE TOE PORTION OF FOOT.

WHILE MINER WAS HOOKING UP LIFTING STRAPS TO LIFT AND REMOVE DISCHARGE SHUTS ON PLANT FOR REPAIR, MINER SLIPPED ON A LOOSE PART OF WORK PLATFORM RESULTING IN A FALL OF A DISTANCE OF 5 FT ONTO A CONCRETE FLOOR. AS A RESULT OF FALL THE LEFT ELBOW, LEFT SHOULDER AND LEFT RIB CAGE OF MINER WERE INJURED.
