Showing all 6Replaced roller on conveyor - extended arm to drop roller in and felt pain.
EE was cutting a piece of electrical conduit with a portable band saw. As the cut was nearly complete, the part shifted and moved the saw blade into EE's left index finger. The cut required suturing.
EE was sweeping gravel into a shovel held by a co-worker. When the shovel became full, co-worker lifted it up hitting EE's right hand. The shovel struck EE's middle finger causing laceration. The co-worker, a certified EMT, cleaned, applied antibiotic ointment, & bandaged the wound. EE reported this 3 months later & wanted to have it looked at by a doctor.
EE returned to his area (Primary Plant) after lunch and radioed in to startup this equipment. He removed a guard at the tail of the 01-CY-02 and reached into the belt transition area where he was grabbed by the left arm between the first trough roller and the conveyor belt. He radioed in STOP! and said he thought he may have broken his arm.
EE was repackaging a hydraulic cylinder. As he slid the packing head onto the rod, he got his right middle finer pinched between the head and the barrel. This caused his finger tip to get cut also. He went to the doctor to get medical treatment. The doctor treated his finger, which required three sutures. EE returned to work after treatment.
