Employee was running a grader. Employees noticed grader was banked against a rock pile and went to see why. EE was found slumped over the steering wheel of the grader. It was originally thought that he suffered from a heart attack, but autopsy shows possible electrocution. Accident is still under investigation through MSHA. Another investigator took over the investigation.

AN ACCIDENT CONTRIBUTING TO THE EMPLOYEE'S CONDITION IS UNKNOWN. EMPLOYEE OPERATED A VOLVO L220D RUBBER TIRED FRONT END LOADER. THE LOADER IS EQUIPED WITH AIR SUSPENSION SEAT, A/C & ADJUSTABLE STEERING & OPERATING LEVER POSITIONS. 4 TO 8 HRS/WK EE WAS ASSIGNED MAINTENANCE DUTIES SUCH AS WELDING, MECHANICAL REPAIRS TO CONVEYORS, OIL & GREASE.
