Showing all 9Employee was running a grader. Employees noticed grader was banked against a rock pile and went to see why. EE was found slumped over the steering wheel of the grader. It was originally thought that he suffered from a heart attack, but autopsy shows possible electrocution. Accident is still under investigation through MSHA. Another investigator took over the investigation.
Was walking down stairs by office, got stung 2 times in the left hand by yellow jackets.
AN ACCIDENT CONTRIBUTING TO THE EMPLOYEE'S CONDITION IS UNKNOWN. EMPLOYEE OPERATED A VOLVO L220D RUBBER TIRED FRONT END LOADER. THE LOADER IS EQUIPED WITH AIR SUSPENSION SEAT, A/C & ADJUSTABLE STEERING & OPERATING LEVER POSITIONS. 4 TO 8 HRS/WK EE WAS ASSIGNED MAINTENANCE DUTIES SUCH AS WELDING, MECHANICAL REPAIRS TO CONVEYORS, OIL & GREASE.
EE STATES HE WAS OPERATING WATER TRUCK POSITION. TRUCK UNDER LOADING SPOUT, STOPPED TRUCK, JUMPED OUT TO REFILL TRUCK, THAT'S WHEN HE FELT SOMETHING POP IN HIS RIGHT FOOT.
COWORKER & EE WERE PASSING EACH OTHER ON HAUL ROAD, EE WAS EMPTY & COWORKER WAS LOADED, ROCKS CAME OFF COWORKER'S TRUCK & BOUNCED OFF THE TIRES & HIT EE'S WINDSHIELD CAUSING IT TO SHATTER. EE HAD A FEW GLASS SPLINTERS IN HIS RT EYE. ANOTHER COWORKER TOOK HIM TO ER. GLASS SPLINTERS REMOVED & EE REC'D TETANUS SHOT & ANTIOBIOTIC EYE DROPS, RELEASED TO FULL DUTY NEXT DAY. FOLLOW UP APP

EE WAS DRIVING 6-WHEELER OFF HIGHWAY END DUMP, 30 TON, WENT AROUND CURVE TO EAST AND BED LAID OVER ON SIDE BRINGING TRUCK TO ABRUPT HALT. KNEE HIT DASH.
