A contract mechanic was driving a pin with a sledgehammer on the tracks of an excavator when a piece of the pin broke off and embedded in their thigh. Mechanic was taken to the emergency department where they received stitches for the injury. They missed one workday and returned the following Monday.

Showing 50 of 67Miner was injured while using a scaling bar to remove loose rock while loading a header. The bar glanced off the rock and hit EE on EE's right thumb. EE was taken to be evaluated and was diagnosed with a contusion and placed on restricted duty. Restrictions do not prevent EE from driving a truck, which EE has been doing without incident.
EE lost EE's balance while loading a floor shot and hurt EE's knee. EE was taken to the Occupational clinic to be evaluated and was diagnosed with a left knee sprain. EE was placed on restricted duty until EE's follow up appointment on 9/5/25.
The miner was adjusting a conveyor belt. Miner was injured when the pipe wrench miner was using slipped & mashed finger. Miner was wearing ppe. Miner was taken for a medical exam & diagnosed w/ a contusion but only received 1st aid treatment. Miner was restricted from lifting weight with hand. On follow-up appt. miner was told to only work in a splint & would be reevaluated.
The miner was injured prior to loading explosives at a header. EE entered the basket on the powder truck and was manipulating the ANFO hose when the hose "whipped" and struck the face, causing small pieces of rock to break off. One piece struck EE in the right palm causing a laceration.
EE was exiting from EE's truck at the end of shift, EE missed the bottom step causing EE to fall in the ten pile hitting the back of EE's head.
Employee stated they had clocked out and as they were leaving the office, they fell going down the concrete steps outside the office landing on both knees.
EE was cleaning EE's windows and mirrors on the HM300-5 articulating truck, when EE started to come back down the steps, EE missed a step causing EE to fall landing on EE's back and shoulder.
EE dropped EE's flashlight on the ground, when EE bent over to pick it up EE lost EE's balance and fell face first on the gravel.
EE was riding on the loader with EE's legs hanging off the front of the loader to the backside of the mine to get a pickup truck. The loader driver went to make a left turn and when EE did the step on the front of the loader caught EE's left leg.
EE WAS TAKING DRILL STEEL APART OFF THE FLOOR DRILL, THE STEEL SWUNG AROUND STRIKING THEM ON THE TOP OF THE LEFT HAND.
EE was climbing down off the WA 500. Misjudged the steps and fell hitting the back of head on the ground.
The employee was removing the 4" dust suppression hose that was stopped up on HD-943 drill.
EE was weed eating the bank and there was loose rock in the area, EE was starting to walk up the bank and slipped on the loose rock, landing on their right knee.
EE was operating the floor drill when a rock slid off the lower half of the rib and pinched the dust hose. EE was attempting to move the rock off the hose when the rock shifted causing EE's right elbow to strike the rib.
EE was working on a hydraulic hose on CH-294. EE was cutting hose wrap from the hose, in a tight space & the razor knife slipped and cut EE's index finger on the left hand.
Employee had lost the tooth off the pick and was trying to recover it from near the face. While picking up the scaler tooth a rock fell off and struck the employee in the right shoulder.
EE was shoveling under the crusher, when EE turned EE's body while shoveling, EE felt EE's back pop.
EE was helping carrying a screen to be changed out, and stepped into a 5" opening at the edge of the plywood and screen.
EE was cleaning under #7B belt using RL-994, RL-994 slipped & sunk in the mud, this caused the back end to raise up and pin the EE hand between RL-994 and the belt frame. EE was wearing all PPE.
Miner was operating a ridged frame haul truck. EE was being loaded at the mucking pile and was jostled in the cab bumping head. Miner said ee had seat belt on at the time.
EE was working at the wash plant, the plant stopped up & EE was climbing over the conveyor to clean it out. While climbing over EE heard their Right knee pop out of place.
EE was helping unload the crane off the lowboy being delivered to the Quarry.
Employee was loading plates onto the roof bolter, when they slipped and fell onto their pinky finger.
EE REMOVED THE GEAR BOX FROM C3 WHICH WAS ATTACHED TO THE CATWALK WHEN IT WAS REMOVED IT LEFT A HOLE IN THE CAT WALK AND EE STEPPED IN THE HOLE.
Employee was operating the excavator, when it became hung up on a large rock, the rock broke loose causing the excavator to drop back on the ground jarring the employee.
EE placed EE's hand on the tail pulley to turn it in order to access the set screws. Other employees were helping, and one employee turned the pulley at the same time resulting in EE's left hand being cut.
*This was not an employee* A customer truck entered the yard, the driver exited the truck and advised the scale house they were having chest pain. At that time 911 was called along with an EMT on the job. Customer was transported to the local hospital.
EE was running the anfo hose on the powder truck loading a shot, when EE felt a pain and a pop in EE's right forearm.
EE was hammering on a piece of metal, trying to straighten it out. EE missed swinging at the metal and strained EE's right wrist. An incident report was filed on but no medical attention was administered. EE aggravated the wrist again on 3/12/18, and sought medical attention on 3/13/18.
Cold weather caused the bins to stop up EE was using a long bar to dislodge material into the truck bed. Shortly after EE complained about muscle spasm in bicep of right arm.
Employee was changing out a bearing on the 7A belt, Employee was working out of a man lift basket, Employee bent down to pick up a tool and had a catch in lower back.
Miner was welding on a piece of steel in the shop. EE removed gloves and walked around the welding table to reposition leads. Then went back to the steel to slide it around and burned hand on a hot weld.
EE WAS CLIMBING DOWN LADDER FROM BELT TO GROUND EE'S FOOT SLIPPED & EE CAUGHT SELF OVERHEAD WITH RIGHT ARM,
EE WAS OPERATING THE ROAD BROOM AND GOT OUT TO TURN THE WATER VALVE ON TO THE BROOM AND EE TWISTED LEFT KNEE. WAS TAKEN TO INSTANT CARE WHERE THEY WERE TOLD THEY STRAINED THEIR MCL AT THE KNEE, RETURN TO WORK WITH RESTRICTIONS.
EE was checking radiator on excavator when the hinge broke and the hood fell, hitting EE on left shoulder blade.
EE was installing new conveyor belt. While cutting old belt off his utility knife slipped and cut his right hand between thumb and index finger. EE received stitches at ER.
EE was complaining of chest pains
EE was unstopping #5 bin with a pipe. EE came in to work the next morning complaining of pain between his shoulder blades.
EE was driving when he started feeling light headed and dizzy.
EE was walking down steps when his foot slipped causing him to fall onto his right shoulder. Causing contusions to EE's right shoulder and neck area.
EE was checking room for loose rock. He started to pull a loose rock when the one above it fell hitting EE on the left hand and arm.
EE was raising manlift to get to feeder pan when he caught his right index finger between structure and basket. EE stated that manlift traveling after he let off of controls.
EE was climbing up on CH-368 to make repairs. Lost his balance and fell off. Twisted his right ankle.
Roof fall no injuries
Roof fall over the weekend - no one on site.
EE was on drill to remove radiator when he slipped and fell approximately 6-7 feet EE broke both wrists
EE had hand scaled area prior to loading shot. Rock fell from top of face and hit EE on the left side of head/hard hat then onto left shoulder. Approx size of rock was 6-8 inches in length and 3 inches thick.
EE was cutting light gauge stainless steel with a grinder, EE held the material improperly grider kicked up and the blade hit the inside of his forearm.
Wrecking forms away from concrete, crow bar slipped and just had a light sprain to left hand wrist.
