While attempting to move a contractors trailer, a Plant Maintenance EE was cranking up a tongue jack that was very stiff to turn. After EE was done, EE noticed that EE had some pain on the outside of the right elbow.

Showing 50 of 248Employee was cleaning the slurry pit when EE's right leg sunk into the ground up to the knee. The employee pulled EE's leg out of the ground and EE's leg was covered with slurry. The employee washed off with a nearby water hose but forgot material under EE's sock and suffered chemical burns on EE's right foot. The employees wound got infected and EE was prescribed antibiotics.
The employee was the passenger of a gravely (UTV) riding along when the vehicle had a minor backing incident into a wooden post. The driver of the vehicle was fine but the passenger stated had a headache after the incident and requested to go home.
On July 24 at 0415hrs a production employee was tasked with cleaning out south 4 baghouse compartment. As the EE was attempting to open the second clamp on the compartment hatch material came spilling out onto employee knocking EE's safety glasses down on EE's face and getting into both eyes. This incident caused chemical burns in the left and right eye of the employee.
Plant Maintenance employee was trying to free up a preheater ram/block that was not moving with a come along to asist the hydraulic pressure of the ram cylinder. While doing so the come along broke off the hooked end and struck the employee in the right hand.
On 10/17/22 around 5:40am, employee was walking to the shipping office to clock in for work when slipped and fell on right side injuring shoulder. The area that the EE was walking through was wet from the rain during that time. The slip & fall resulted in the employee dislocating the shoulder and having to be put on restricted duty.
Employee was performing an inspection of a conveyor belt. As they were climbing down a ladder they stepped in mud build-up twisting their foot and leg. The twisting motion caused them to temporarily lose footing and employee felt a pain in their back.
While loading slurry into a tanker truck, slurry coming from a leaking hose splashed into employees face getting behind the lenses of the safety glasses causing a chemical burn in employee's right eye.
After an inspection of a kiln, Employee was using a blow off hose to clean self off. While cleaning off the employee accidentally dropped the nozzle to the hose and when EE bent down to pick it up EE stated that EE heard something pop.
Employee(a) worked on 11/10 from 0730-1630hrs, after working EE's scheduled shift employee(a) was called back to work at 1900, 2218, 0134 and 0230hrs. On 11/11 around 0430hrs employee was replacing a gearbox with employee(b). While employee(a) was aligning bolts for installation, the gearbox slipped out of employee(b) hands and crushed the pinky finger of employee(a) right hand.
Employee was working on a screw conveyor motor and the wind blew lime dust in between the frame and side shields of the safety glasses into employees left eye.
Employee was inserting a long handled sample scoop through a small window on Kiln 1 to obtain a sample of lime from the kiln. While performing this task lime dust and debris flew into employee's eyes.
While employee was working on a water pump EE realized the system was not pumping water properly. While the employee was removing the hose connection to the water pump EE was sprayed with hot water. Employee suffered second degree burns on EE's right arm, chest and a small area on EE's back.
Patching roof holes, setting plates down and rib pop out of place, left side, aggravation of an old injury.
Employee was was on a call out. When EE was squatting to walk under the screw conveyor EE's forehead slammed into the gate. EE was wearing a small brim hard hat turned around with the dial in the front. The dial hit against the structure. The force of the blow resulted in a concussion.
Employee was walking from the restroom toward the breakroom and suffered a fall with uncertain cause. The impact against the cement floor caused laceration to eyebrow and open nasal fractures.
Employee was helping a welder by holding the metal pieces while the welder tacked the pieces together. During the welding process, employee looked at the arch flash, which caused cornea flash burns to both eyes. Requested medical visit on 1/21/2021 and was prescribed medication for eyes.
Employee was walking near the Ball Mills when EE stumbled and fell backward on to the ground. EE's left elbow scraped a cement pad causing a laceration that required sutures to close.
Employee was installing and aligning the base for a gearbox and when sliding the base into position, employee's middle and ring fingers on right hand were pinned between the base and a piece of metal causing a laceration and fracture.
Employee was standing on the platform to the Pulverized Mill spotting crane. Material fell from above and got behind employee's glasses and into left eye causing irritation. Medical visit on 09/06/20.
Employee was using a plasma cutter to make a hole in metal. When employee was striking the metal with a hammer to knock out the piece in the hole, employee felt debris fall on face. Employee wiped the debris off and accidentally rubbed it into eye.
Employee felt pain in right elbow while rotating pulley on pulverized whizzer. Pain increased over several weeks. Employee requested medical attention on 3/20/20.
While checking the control panel at the dust collector fan for the Pulverized load out spout. Wind blew lime behind glasses and into left eye causing irritation. Employee requested medical visit on 3/2/2020.
While exiting personal vehicle in employee parking area, employee swung the driverside door open and began to step out of the vehicle. The door swung back and struck employee in the head causing a laceration on the upper forehead which required 7 sutures to close.
Employee was removing lime in the Product Tower. Lime burn developed on wrist. Over the next week, the wound got infected. Medical attention requested on 1/9/20.
Employee was working on the RK3 Pan Conveyor Bucket Elevator when wind blew lime behind safety glasses causing irritation to the left eye.
Employee was walking down a set of stairs leaving while carrying a bucket of tools. While walking down the steps, EE's knee began to hurt. The pain got progressively worse over the next couple of days and EE requested medical treatment on 10/25/19.
When employee connected 2 railcars together, the force of the connection caused employee's head to jolt forward and strike mouth on frame of open window causing a laceration to the upper lip which required stitches.
Employee was using a wheelbarrow to remove spilled material and slipped/tripped which caused the wheelbarrow to tip over and spill material into their face causing the eye to become irritated.
While in an awkward position, an Employee was pulling a transmission jack by the handle and developed pain in their lower back.

Employee was trying to climb up a small ladder while carrying a 44 lbs motor in EE's hands and lost EE's balance. The motor slipped and pinched EE's hand between the ladder and the motor causing a contusion
Employee slipped going down stairs falling backward and striking elbow and other parts of body. Requested medical attention on 3/7/19 for foot sprain.
Employee was brushing dust off the front of shirt and as ee swiped right hand down left shoulder/chest area, the ridge of palm got jabbed by a the tip of a pencil in front pocket. Medical Visit on 10/24/18. The puncture wound got infected requiring medical intervention to drain the infection on 10/26/18.
Employee was working in a kneeling position. When ee went to stand up, ee wrenched back causing pain to develop.

Employee was inside of kiln #3 standing on a wooden board work platform in order to descale the side of the kiln. One of the planks broke. Employee fell approximately 3 1/2 feet landing their back on top of rocky scaled material causing a fracture to a vertebrae.
Employee was installing a screw in an electrical panel. The screw contacted a power cable in the panel and created an arc flash which burned employee's left thumb and forefinger.
