Showing all 3EE was walking to the office to punch out and leave for the day, when he slipped on loose and slippery ground, causing him to fall. EE tried to catch himself and he fell with all his weight onto his right arm, which caused two fractures above the wrist. EE was taken to the E.R at Hospital for care.

A step ladder was used instead of a straight ladder. Ladder was not secured. Working alone on a two man task. Not following instructions. Injury: Bruised, left side of upper body.
