Showing all 22Injured EE(1) instructed EE(2)(Tower Operator) to bump the conveyor on to dislodge a rock. EE(1) had gone back to the tail-pulley and after the "bump" saw a rock and reached in to remove it, at that moment EE(2) "bumped" the conveyor again, pinching EE(1)'s left index finger in the pulley.
Cleaning threads on a bolt using an impact-wrench and an open-end wrench along with penetrating oil when open-end wrench slipped in hand under power from impact wrench striking the face
Employee was walking down steps, missed a step, grabbed with left hand and strained left shoulder. No lost time on medical diagnosis until 1/2/2024 when they went on light duty. On 2/6/2024 had an MRI and was diagnosed with left pectoral tear and opted for surgery.
Pulling large spring loaded lever to disconnect power to Crsh020 when hand slipped off lever striking employee in the bottom of the chin.
CARELESSNESS WITH A RAZOR KNIFE
EE was cleaning EE's air Filters while dismounting on the back rear step, EE misplaced EE's foot on the step and got EE's lower leg stuck between the step and machine.
Was changing cutting edge on a bobcat WL130 put EE's hand under bolt to engage in lock when it spun and it cut EE's index and mid fingers. 12 stitches were necessary to fix.
Employee parked the loader, dismounted and stepped into an impression in the ground. Employee twisted left ankle.
Cutting conv. belt with razor knife. Knife slipped and cut leg. Made small cut on left leg. Dr. put one staple in it.
Employee was checking breaker box. It blew up on him.
While cutting a piece of conveyor belt with a box knife, the knife slipped and cut his arm requiring seven stitches.
While putting the bed pin into an end dump truck, the injured lacerated his finger requiring 3 stitches.
Employee was changing screens and bumped his elbow. Employer was notified on 7/19/07.
A boulder got stuck in the jaw crusher. Employee attempted to pull the boulder free with a log chain and loader. When employee attempted to hook the chain, the boulder slipped, pinning him between the jaw and the boulder.
Employee was going up steps at the jaw. A rock rolled down the jaw belt and fell off the belt. The rock hit the employee in the back.
The employee was welding on a plate of steel. He leaned on the plate and got a splinter of metal in his elbow. He removed it with his finger. There were no witnesses to this incident, and the employee did not report it to his supervisor until 10/13/05.
While helping another ee to raise and lock up the hinged radiator on a skid steer loader to allow adding oil to the engine, it accidently got dropped onto the injured'd finger and pinched it. The finger was bandaged by the doctor. No stitches or broken bones, just lost time and prescription.
WHILE OPERATING A 773 BOBCAT TO HELP FREE A FORKLIFT THAT WAS STUCK, THE INJURED STRAINED HIS BACK REQUIRING A PRESCRIPTION FOR MEDICATION AND ONE DAY MISSED WORK.
While descending the stairs, he missed a step and slid on his backside until he came to rest at the bottom.
After filling water truck, employee slipped while climbing into truck and injured left ankle.
While traveling up a ramp on a crane, ee lost traction and coulnt regain it. Crane was backed 20' into the berm & left rear wheel rolled up on the berm tipping the crane over and threw ee thru the window open top cab roof that was in an open position. crane is a front wheel drive with non rops cab and no seat belt
