EE picked up a piece of scrap metal and threw it into the scrap metal bin and it cut EE's hand.

Moving an air compressor and the air compressor was dropped in the process.

Our technician was working onsite at the C&J Gravel shop in Durango, CO, Mine ID#0504034. While removing jaw wedge with an impact wrench the pusher bolts and wedge became free pushing the impact wrench into EE's chest, causing EE to fall on the grizzley fingers. EE was treated and released at Mercy Medical missing one day of work on Friday 1/15/16

He was climbing into the trackhoe and slipped on the step and injured his left knee

EE was using a wrench to tighten some bolts on the crusher when the wrench slipped and he hit his mouth on the crusher itself.

Employee was running a skid steer cleaning around a conveyor belt when the corner of the skid steer bucket hit a tire causing the tire to explode. When the tire exploded some sand was blown into the employee's face and eyes.

He was picking up a metal plate that had came loose off of the jaw crusher, while picking up the plate he pulled some muscles in his back. He thought as time went by the muscles would get better but they just got worse.

While he was cleaning out material buildup on a chute, which is suspended by chains and one of the chains broke causing him to fall against the conveyor railing frame and he bruised his left knee.

After servicing the loader, EE started walking to personal truck to go home and stepped wrong then slipped on snow. (It had been snowing all day) resulting from the slip & twisting of his ankle he fell to ground onto his right side. He said when he slipped he felt something pop. EE said he was going to wait until morning to have Dr Check ankle.

Carried a heavy roll of vapor barrier from pickup to office, was squatting to set roll down on the floor and dropped roll on floor. Next day was experiencing some pain in groin area. Two days later was not able to walk very easily.

He was putting oil into the grizzly feed to one of our rock crushers when he turned to come down the pile of dirt he was on he fell. He tried to catch himself and hurt his back.

Employee was heating a large bolt that was stuck. A chain holding the cone head in place broke, allowing it to turn, causing him to slip and fall approx. 2 feet.

While cooling a bearing race with propane, another employee was heating the bearing boss with an open flame, approx 30 ft away. Propane fumes ignited. Employee was burned by flash on right arm.
