The miner was doing a plant startup when they noticed smoke from the screen tower. The miner proceeded toward the screen tower to inspect the problem and fell on loose material and injured their shoulder. Miner then went to the doctor and did an x-ray / MRI resulting in a torn rotary cuff.
EE was stocking out material from a series of bins. EE was returning to his truck from the loading area, as he proceeded down a catwalk, three steps and onto level ground. He tore his quadriceps muscle as he was walking down the steps.
EE was climbing into bed of triaxle, reaching on top of bed with hand. Lost grip and fell striking another truck, ground.
EE WAS ATTEMPTING TO PLACE A MOTOR SHIEVE IN PLACE FOR STORAGE. THE SHIEVE ROLLED AND SMASHED HIS FINGER BETWEEN THE SHIEVE AND TIRE RACK. THE FINGER REQUIRED STITCHES.
THE OUTSIDE TEMPERATURE WAS @ 20 DEGREE. THERE WAS SNOW AND ICE ON THE GROUND. AND ON THE EQUIPMENT. EE WAS EXITING THE SERVICE TRUCK WHEN HIS FOOT SLIPPED. HE FELL TO THE GROUND ONTO HIS HIP. THE DR DETERMINED THE ONLY INJURY WAS A FRACTURED HIP.
