Showing all 3Employee stepped down from drill cab, released grip on hand hold as left foot touched ground, left ankle rolled over on a stone causing him to lose balance and fall landing on left shoulder. Left ankle swelling caused difficult walking. Left shoulder popped as it hit ground, causing it to be very sore and unable to lift arm without pain.
RIGHT HAND FINGERS #3, 4 & 5 WERE LACERATED, 19 TOTAL STITCHES, AND FINGER #4 WAS FRACTURED CAUSED BY PINCHING IN BETWEEN TWO 250 LB FEEDER PLATES. WHILE PRYING INTO POSITION, A PLATE MOVED IN UNEXPECTED DIRECTION SUDDENLY. EE HAD JUST WARNED CO-WORKER TO KEEP HIS HADN OUT OF PINCH POINT.
