A large flat stone became lodged at the transfer point between the scalping screen chute & picker belt. EE attempted to dislodge the rock from the walkway beside picker but was unable to remove it. He moved around to the end of picker belt & used a spud bar to try to remove the rock. He jerked on it. The bar slipped, he lost his balance & fell to the concrete floor below.

SEVERAL EE WERE CHANGING SCREENS. THEY PLACED A NEW SCREEN ON TOP A HANDRAIL IN ORDER TO PUSH SCREEN OVER THE SIDE. NO ONE WAS OBSERVED IN THE AREA. HOWEVER THE INJURED PARTY CAME DOWN THE STE PS UNDER THE SCREEN & WAS STRUCK ON THE FOOT. PIECES OF BELTING LYING ON THE PLATFORM HAD OBSTRUCTED HIS VIEW OF THE AREA.
