EE was inspecting the portable crushing/screen. EE's hand was resting on the I-Beam when a rock struck EE's finger tip resulting in a laceration. The rock fell from the screen above approximately three feet striking EE's finger tip.

Showing all 9At approximately 11:30 am ee received an electrical shock. EE was standing on the second step up from the ground of a 6' aluminum ladder which was leaning against a conveyor belts metal tail pulley guarding. He was adjusting a nearby water valve with his right hand as he was holding the guarding with his left hand.
EE WAS ADJUSTING THE ELEVATOR CHAIN ON A WHEEL SCRAPER WITH ASSIST FROM A CRANE. THE LIFT CHAINBROKE, AND HIT EE ON THE HEAD, CREATING A CUT TOTHE TOP BACK RIGHT SIDE OF THE HEAD. IT REQUIRED 10 STITCHES & NEEDED TO BE KEPT CLEAN
WHILE INSTALLING HEAD PULLEY IN FRAME, PULLEY SWUNG AND BRUISED SHIN.
WHEN CLIMBING INTO DRIVER'S SEAT ON MOTOR GRADER, EMPLOYEE WAS ONLY HOLDING ON WITH ONE HAND-LOST HIS FOOTING AND CAUGHT FINGER IN FRONT WINDOW FOLDING BRACKET.
PLACING SUPPORT DOLLY UNDER STREET BROOM SUBFRAME THUMB WAS PLACED ON TOP OF SUPPORT DOLLY WHEN SUBFRAME SLIPPED & FELL ON THUMB
OBSERVING BLEEDING PRECAUTIONS FOR HIGH PRESSURE SYSTEM, REMOVED FITTING OIL SHOT IN FACE HE INHALED FLUID IN LUNGS
EMPLOYEE WAS INSTALLING SEAT IN LOADER. WHILE LIFTING, HE FELT A PULL BUT DIDN'T THINK IT WAS SERIOUS. IT GOT WORSE EVERY DAY SO HAD TO SEE A DOCTOR.
EMPLOYEE WAS UNSCREWING A PISTON BOLT ON A TRUCK. THE BOLT BROKE, CAUSING HIS HAND TO HIT ANOTHER BOLT ON THE MOTOR. THIS RESULTED IN A CUT ON HIS LEFT MIDDLE FINGER WHICH REQUIRED THREE STITC HES.
