Employee was changing screens. He was climbing down a ladder and had reached the second rung from the floor. He decided to jump the rest of the way. His glove hung up on a bolt and he was jerked. The employee began experiencing pain in his lower back.

Employee was spinning anvils in a Vertical Shaft Impact Crusher. The lid of the VSI came down upon his left hand. Employee's left hand was caught between the lid and frame of the VSI. The ring finger of his left hand was seriously injured.

Employee was using an impact wrench to loosen the bolts on the cutting edge of a loader bucket. He reached under the edge of the loader bucket to locate the bolt head. He then engaged the impact wrench. The bolt head on the underside of the loader bucket spun, caught the employee's glove, and lacerated his left middle finger.

EE WAS BACKING A WHEEL LOADER FROM A STOCKPILE. THE RT. REAR TIRE ROLLED INTO A GROUND DEPRESSION. THIS CAUSED THE WHEEL LOADER TO ROCK HARSHLY FROM SIDE TO SIDE. EE EXPERIENCED PAIN IN LOWER BACK IMMEDIATELY AND PAIN WORSENED OVER THE WEEKEND.
