Filling water truck. When full, EE walked to shut water off. On the way, EE walked under sand plant. EE came to a 4 inch water pipe, 19 inches off pad. Instead of stepping over pipe, EE stepped on it and fell on right shoulder.

Showing all 4Taking dredge pump apart, leaned face plate on a beam. When they put pump back together, employee stepped back and hit the face plate. It slid down right leg.
Employee was making dredge repairs using hand tools: 3/4" ratchet & 4'-long cheater pipe. He took dredge pump apart and left the ratchet and pipe on the end of the shaft. He started the engine and then engaged the clutch. The pipe came around and hit him on the left side of his skull. Employee was hospitalized and died of his injuries on 7/3/05.
WE WERE GETTIN GREADY TO CHANG A 40 FOOT PIPE ON DREDGE PIPELINE. IT WAS RAINING THE PIPE WAS SLIPPERY. HE WALKED ON TO TH EPIPES WHEN HE SLIPPED AND FELL ONTO THE PONTOON. HITTING HS RIGHT KNEE ON A BRACE CUTTING IT. NEEDED 7 STITCHES.
