Employee was responding to a fire at Screen Stone, he jumped out of the truck and grabbed two fire extinguishers, as he ran to the stairs he slipped in the mud and landed on his butt and back. Incident was not reported until 11/06/2014 because at the time he felt no injury.

Employee was sorting through a box of paperwork when a piece of paper fell on the ground the employee bent over to pick it up and his back went out causing him to fall forward and hit his head on the door. Employee was sent to the doctor for evaluation. No damage or impairment was caused.

Working on a drain pipe and a swarm of bees erupted. Person was swinging his arms to keep the bees away and heard a loud pop in his shoulder. This person has been put on light duty due to both injuries

Employee bent down to turn on water valve located approximately one foot from the ground. Twisted or tweaked his back in the process. EE is currently off work pending further medical evaluation and release from multiple doctors restrictions.
