EE was using CAT Backhoe to clean up area, removing scrap conveyor belt. He scooped up some belt with the bucket of the backhoe and got out to push the remaining into the bucket.

Daytime- Sunny- Off road conditions.

Working on the counter weight belt B4. In process of removing an old guard, employee knocked a piece of steel loose. After being informed that the piece was loose, an approximately 20 lb piece of angle iron came down and hit his hand.

Employee bent over to pick up a torch and his back locked up.

Putting 10' Length of 2" conduit in storage rack approx 35lbs to 45 lbs.

Walking by a trailer employee was started by a cat coming out of the weeds and tripped and fell on his leg breaking it.

EE was installing a new grounding rod (3/4 copper) with a hammer. He missed the piece his hand took the impact slicing his left palm.

Employee got out of his bulldozer to move a stacker. He attempted to cross a retaining wall and fell about 3 ft.

Employee working in possible 'bite' area if steel should break away from it's mount while forcing with come-along. Damage was to one of the employee's knee (right)

EE strained a muscle while moving sand from a belt. EE notified management on 01/11/2010 of injury and we started an investigation, and sent him to be examined on this date.

Pushing hand truck and slipped on gravel.

Employee slipped on ice.

Felt a groin pull when crossing from barge to ship.

Injured was attempting to clear blocked transfer chute when a large rock fell pinning his hand against a steel skirt board.

Employee was pulling on a crowbar and it slipped and hit him in the face.

Employee broke bolt loose and it snapped, hitting him in the eye under safety glasses.

Hernia--EE notified not to do any heavy lifting.

Employee was pulling on a pipe wrench towards his face and wrench slipped off & struck him in the face.

Slipped going through engine room door.

Employee picked up a 50 pound pump and threw his back out.

CONVEYOR BELT ROLLER FELL ON EE'S HAND.

Picking up plates hand got wedged between plate and concrete.

Pulled shoulder lifting a board.

EMPLOYEE WAS ADJUSTING CONVEYOR BELT TENSION WITH MANUAL COME-ALONGS. BELT CAUGHT ON A PIECE OF THE FRAME AND PARTED. EMPLOYEE WAS THROWN INTO A STEEL ANGLE.

EE BACKED A BOBCAT HE WAS DRIVING INTO A POLE. EE SAID HE WAS INJURED ON 8-14-03 BUT REPORTED INJURY ON 8-17-03.

EE WAS CARRYING A RAILROAD TIE WITH ANOTHER EE WHEN IT SLIPPED AND FELL ON HIS FOOT. EE WAS WEARING STEEL TOE SHOES BUT RAILROAD TIE HIT THE UPPER PART OF HIS FOOT.

EE STATED HIS ANKLE WAS HURTING AND IT MIGHT OF BEEN CAUSED BY MOUNTING OR DISMOUNTING HIS MACHINE.

EE WAS WORKING MOVING OXYGEN BOTTLES HE WAS INSTRUCTRED TO GET HELP UNLOADING THE BOTTLES. APPARENTLY HE LIFTED BOTTLES HIMSELF
