Wednesday, 11/09/2011, the individual was inside the screen pushing up on a panel and felt pain in his shoulder. He felt the injury was insignificant and didn't report it to management until 11/14/2011. He was referred to his primary care physician immediately who felt that it was minor.

WHILE BRINGING THE DREDGE IN TO THE BANK FOR THENIGHT THE SPUD HIT A SAND BAR AND CAUSED THE DREDGE TO STOP SUDDENLY. THE BARGES SHIFTED CAUSING THE CABLES TO SNAP TIGHT. EE WAS ATTEMPTING TO GET AWAY FROM THE TIGHTENING CABLES WHEN HE SLIPPED AND FELL ON HIS RT. ARM.
EE WAS USING A SLEDGE HAMMER PUTTING METAL ONTO A BARGE DECK.
EMPLOYEE WAS MOVING A 3" PUMP AND STRAINED BACK.
EE WAS REMOVING BOLT ON LOADER BUCKET WHEN HIS HAND SLIPPED CAUSING A LACERATION TO HIS RIGHT INDEX FINGER
EE WAS ENROUTE TO THE DREDGE WHEN A PIECE OF DEBRIS BLEW INTO HIS RIGHT EYE.
EE WAS BENDING OVER TO PICK UP BRAKE SHOES THAT WERE LYING ON THE FLOOR OF SHOP. WHEN HE TRIED TO STRAIGHTEN UP HE WAS IN PAIN IN HIS BACK.
