THE EMPLOYEE WAS GETTING ON A D9 CAT DOZER. THE EMPLOYEE'S FEET SLIPPED OUT FROM UNDER HIM AND HE FELL TO THE GROUND. HE FRACTURED HIS RIGHT ARM ABOVE THE WRIST. EMPLOYEE HIMSELF SAID ACCIDENT WAS DUE TO SLICK SOLE WORN OUT SHOES.
