Highwall failure. 7/23 4:30 pm - 5:30 pm small amounts of rock started to fall to the existing bench. 7/23 5:30 pm - 6:00 am on 7/24/12 large amounts of rock from highwall fell.

Showing all 6While on vacation EE contacted cold. Cold went away accept for a small lingering cough miner developed chest pains went to Dr. and was diag. with pneumonia. Doctors changed diagnosis to possible Valley Fever. No Eq. was involved leading to illness.
Operator was walking from dozer to fuel truck and tripped over a dirt clod and twisted ankle resulting in broken bone in ankle area.
Driver pulled into safe area to allow another truck to pass. When driver backed out of safety zone, he backed up too far resulting with wheels over embankment causing rollover.
Mechanic was changing cutting edges on loader bucket. While attempting to get large bolt in correct placement, he accidently activated air impact wrench which spun bolt and caused a laceration of left wrist requiring stitches.
EE WAS WORKING ON EXCAVATOR BUCKET REMOVING ROCK TEETH WHEN A SMALL PIECE OF METAL CAME OFF THE TOOTH FROM THE HAMMER BLOW AND LODGED IN EE'S RIGHT THIGH. THE ACCIDENT INVOLVED MINE EQUIPMENT PARKED AT THE MINE SHOP FOR REPAIR. THE MINER HAS LOTS OF EXPERIENCE, TRAINING & APTITUDE. NO RULES WERE BROKEN.
