EMPLOYEE TRIPPED AND FELL ON ROCKS INJURING HIS HANDS. CUTS AND ABRASIONS
Ee was installing a rear end in a rock truck. All this pushing and pulling hurt his back. He can't have an MRI done because of medical reasons, so he doesn't know exactly what is wrong in this low back. Physical therapy seems to be helping, but its really too early to say.
Ee is a dozer operator. He back over a rock that was not in his eyesight causing a jolt of the machine. This jolt caused him to be jarred severely causing a hurt back. He missed two days and is not back to work with very little problem.
Ee was dismounting backhoe and suffered extreme pain in groin. Went to Dr, who said he had a hernia. Went to a surgeon , who said he had a hernia. He worked until 2-3-11. Had his surgery on 2-4-11 and is now off work for a while.
EE was helping the powder crew load a hole. As he was putting a wet bag in the hole, he felt something give in his groin. The diagnosis is hernia and he will have to have surgery to repair.
EE was assisting a mechanic. He was using a pry bar when it slipped, hitting a metal surface and pinched the end of his thumb off.
EE is a mechanic. His service truck had broken down. He was under the truck removing the drive line when dirt fell into his eye. First aid was administered. He went to the ER and eye doctor. No damage to eye. Returned to work after one day off.
EE was operating a haul back truck. It hit a rough spot in the road which jerked him sideways in the cab. This caused a strained back. Two days off and physical therapy has allowed EE to return to work with no problems.
EE drives an off road dump truck. When he was dumping his load, the jack failed and the bed came down hard causing him to bounced in his seat. Took one day off and then came back to work, no problems.
EE drives a rock truck at a surface mine. He has expericence back strain over the years. On this date, it became acute and he has sought medical relief.
Ee was using a hand shovel trying to pry a rock from the track on a dozer. He knew his shoulder was hurt immediately. He had to have surgery for a torn rotor cup in his shoulder.
Ee was getting of dozer, slipped and fell hitting head on blade. Minor cut, required only two or three stitches. Ee went back to work.
Ee was operating a grader. He was going up hill when the engine stopped running. The grader then took off backwards down the hill. Ee jumped off. He received only minor injuries, in fact only very minor brusies.
Ee is a truck driver. As he was waiting to get loaded a large rock fell into bed and jarred him while he sat in the cab.
Ee was using a cutting torch when a hot piece of slag rolled into his boot. (He was wearing an open type boot.) By the time he got it off, his leg was burned badly.

EE stepped off loader onto a piece of wood that turned causing him to sprain his ankle.
Truck was being loaded and rocks fell into truck bed hard and threw truck driver against steering wheel
A large rock came from out of the mountains (not from the permitted area) and hit the truck Ee was driving. This knocked Ee from truck causing only bursies and strains. No bones broken. No cuts, only scratches.
Employee was climbing steps to get into his truck; his foot slipped and as he fell his thumb got caught in the steps.
Ee was fueling a dozer when his feet slipped and he fell off the dozer. His left ankle was severely sprained.
Ee was dismounting dozer at end of shift and his knee seemed to be dislocated or pulled.
EMPLOYEE WAS HELPING THE REPAIRMAN TO WORK ON A BUCKET. EE BEGAN WELDING ON THE BUCKET BEFORE HE GOT HIS HOOD INTO THE CORRECT POSITION CAUSING "WELDERS FLASH BURN TO THE EYES".
EE was climbing back onto deck of dozer and caught his boot laces in the battery box causing him to fall to ground.
EE was on his way to pick up a load of rock in his truck, ee lost control of truck and ran into the ditch line injuring his arm and leg in cab of truck. This accident was reported to us on 10-18-04.
EMPLOYEE WAS WORKING DOZER IN ROCK PILE. A DUMP TRUCK WAS UNLOADING WHEN HIS BRAKES FAILED AND HIT THE DOZER KNOCKING EMPLOYEE AGAINST THE SEAT, SCRAPING AND BRUISING HIS LEFT THIGH. THIS ACCIDENT WAS REPORTED TO OUR COMPANY ON 10/18/04.
EE WAS LOADING HIS COAL TRUCK WHEN THE WIND BLEW SOMETHING IN HIS EYE.
ON SAT 1-25-03 EE WAS HELPING TO INSTALL A STABLIZER ARM ON A DOZIER. THIS IS VERY HEAVY AND TAKES FOUR MEN. WHILE DOING THIS HEAVY LIFT IS WHEN THE INJURY OCCURED BUT EE DID NOT KNOW IT AT TH E TIME AFTER A PERIOD OF MISTREATMENT, MRI WAS DONE THAT SHOWED THE DISK PROTRUSION. HE HAS BEEN INT HE CAR OF DR SINCE THAT TIME.
EE WAS GETTING OFF END LOADER. WHILE COMING DOWN LADDER, HE SLIPPED & HIT HIS LEG AGAINST BATTERY BOX. INITIAL TREATMENT WAS FIRST AID. HE CAME TO WORK ON 5-8 WITH NO LOST TIME. WENT TO DR ON 5-13, POSSIBLE BLOOD CLOT, WAS ADMITTED TO HOSPITAL.
EE STEPPED ON BULLDOZER TRACKS TO RETREIVE A GREASE GUN, SLIPPED & FELL TO THE GROUND
