Removing broken master link bolts - D10T track. Elevated portion on the service truck crane and a crib block. When lowering track to ground, engaged boom in order to elevate the track off the crib block. Boom had a delayed due to a now diagnosed missing sensor spring. The boom raised unexpectedly, caused tension on the crane cable and the cable whiplashed. Laceration to forehead.
Employee picked up a bucket of grease to grease the threads on the drill steel on the highwall drill when EE felt a pop in EE's lower back.

Contract security guard employee was found in (personal vehicle) with driver's window down and vehicle running with air conditioning on. Security guard was thought to be asleep but when checked was not breathing and did not have a pulse. This is not due to no accident or injury at the mine site.
Employee was helping set up Highwall Miner when they walked around front left track, when a mud ball fell from highwall and busted on ground throwing debris, hitting employee in groin area.
Employee was greasing track on excavator when the grease hose popped off and struck them on their left hand and caused grease to go under their skin.

Tech was working on rain hoist control valve on 785. When coming from under the truck for more tooling their right foot slipped causing strain to lower right back. Tech did not feel any discomfort in back when they slipped so they continued working. Tech woke up with pain in lower right side of back on 4/13/24.
Employee was operating D6 Dozer cutting a road in at the toe of the valley field when employee backed up the offside track went up a rock and slid off causing it to jar their neck and shoulder area.
Employee was sitting in employee's Mechanics Truck with Mechanics Helper when the Mechanic told the Helper they thought they were having a heart attack. Mechanic helper contacted me on the company radio and I traveled to their location and took the mechanic straight to the hospital.
Employee was operating Caterpillar 777 Rock Truck and was headed back from the dump area through a narrow area in the roadway when another rock truck coming from the pit area also entered the narrow area. Employee hit the brake and caused the truck to slide toward the other truck and employee jerked the steering wheel in the other direction and went into an empty pond.
Employee was using sledgehammer to drive pin in on Highwall Miner and turned causing strain to lower back.
Employee jumped from fuel tank on Mack RD800 Coal Truck to ground spraining right ankle.
Employee was driving down haul road in personal vehicle when employee met an employee from the day shift crew leaving work early coming up the haul road. Employee coming down the haul road was in the center of the road striking the day shift's employee vehicle in the driver's side front.
Employee was getting out of the bed of EE's work truck when EE went to step to the ground from the rear bumper location, employee foot slipped due to the ground being froze and slick causing EE to fall and hitting EE's left knee on the ground.
While pulling a transmission and torque out of loader, had to remove auto lube pump and tank to get out of the way. During the process another employee was helping with it and somehow tripped up and fell causing the auto lube pump to fall and hit employee in the top of head.
Tandem Coal Truck driver was attempting to change to a higher gear on a slight decline. When driver changed gears, EE thought it had gone into gear but was actually in the neutral position. When the driver realized the truck was in the neutral position, the truck had gained speed causing the truck to runaway.
EE was operating 980M Caterpillar Frontend Loader cleaning up loose material in the pit area, when bucket became lodged in solid material. When EE went to back up the bucket became dislodged from the solid material and slammed EE down, causing the seat to bottom out on the seat frame.
Employee was cutting a bracket on the highwall drill with a torch when a piece of slag popped and went through under cutting glass contacting left eye.
Employee was opening side door on box trailer, where wet hole bags are stored. Employee pulled handle to unlatch door when handle sprung around striking employee above right eye, causing a laceration above right eye approximately 1 inch in length. Employee received 4 stitches to the laceration.
Employee was washing floor when EE fell into support beam hurting shoulder.
Handing Mechanic tools over head when a hydraulic block fell on EE's right hand which was on ground.

Tech was using a torch to cut out a tank on a 993K loader. Sparks and spurs came off as tech was cutting landing on pant right pant leg. Tech didn't realize pant leg was on fire until it had burnt tech. Received minor burns on finger tips as tech was putting out the fire on pant leg. Tech had taken off gloves to pat out fire.
Employee was loading mulch on Hydro Seeder Truck when they felt a sharp pain in lower back,
Employee was refueling 777 Truck. When EE took nozzle off of fuel tank, the fuel sprayed out & hit employee in face, the O ring hung on quick connect on fuel tank.
Employee was burning brush pile with an excavator, when wind caught the fire and blew it in the cab with the employee burning EE on left side of face, elbow, leg, & hand.
Employee was pulling a stump out of the ground, when the bucket slid off the stump, made seat bottom out and jerked employee's lower back. Employee did not miss any work until 12/15/2020.
