EE WAS GREASING KNUCKLE JOINT ON CONT MINER AND LIO FELL ON RIGHT ARM AT WRIST.
EMPLOYEE WAS CLEANING LOOSE COAL AND ROCK OFF CONT. MINER WHEN HE TURNED AROUND HE SAID HE FELT A SHARP PAIN IN LOWER BACK.
INJURED WAS RIDING IN SCOOP DIPPER WHEN OPR. RAISED THE DIPPER UP, THE INJURED PLACED HIS HAND TO CATCH HIMSELF AND HIS LEFT MIDDLE FINGER CAUGHT BETWEEN JACK AND JACK STOP.
EMPLOYEE WAS RIDING SCOOP WHEN BRAKE HEAD BLOWED. OPERATOR TURNED SCOOP TOWARD RIB AND WHEN SCOOP HIT RIB INJURED WAS THROWN AGAINST SCOOP DIPPER AND BRUISING BACK MUSCLE.
ROOF FALL IN #2 HDG APPROX. 30' LONG 18' WIDE TAMPERING OFF TO APPROX. 12' WIDE.

INJURED WAS RIDING MANTRIP OUTSIDE WHEN SCOOP DROPPED DOWN IN HOLE ONHAULWAY & BOUNCED INJURED IN BUCKET
EMPLOYEE WAS INSTALLING ROOF BOLTS WHEN A PIECE OF DRAW ROCK FELL AND CUT HIS THUMB.
EMPLOYEE WAS EXITING FROM SCOOP BUCKET WHEN HIS FOOT WENT UNDER BUCKET AND THE OPERATOR PUT PRESSURE DOWN OF BUCKET CATCHING HIS RIGHT FOOT.
WAS MOVING FEEDER FROM TAILPIECE WITH SCOOP AND PULLING WHEN CHAIN BROKE AND THE END OF FEEDER THAT INJURED WAS ON WENT AGAIN THE TOP INJURING EMPLOYEE'S LEFT SHOULDER.
EE WAS PUTTING CABLE REEL CHAIN BACK ON SHUTTLE CAR. HE HAD RIGHT INDEX FINGER BETWEEN REEL AND CHAIN WHEN THE REEL TURNED CATCHING FINGER.
INJ HAMMERING ON BATTERY TRAY WITH 16 LB SLEDGE HAMMER WHEN HAMMER BOUNCED BACK AFTER HITTING TRAY STRIKING ON LEFT LEG.
I NAME WAS PULLING A PIECE OF LOOSE ROOF AND IT SLABBED BACK AND STRUCK HIM ON THE TOP OF HIS FOOT.

THE ROOF CONDITION IN T HE FALL AREA IS FAULTY.
WHILE TRAVELING IN MINE TO BEGIN WORK DAY IT WAS NECESSARY TO TRAVEL UNDER MINER CABLE- IT BEING FASTENED TO ROOF THE INJURED STRUCK HIS HEAD AGAINST THE CABLE CAUSING INJURY TO THE NECK.
STRUCK END OF MINER BOOK CAUSING SUDDEN STOP & CAUSING PERSON TO BUMP HIS MOUTH AGAINST SCOOP DECK.
ROOF FALL ON INTAKE AIR-AREA HAD BEEN DANGERED OFF FOR APPROX 1 MONTH BEFORE THE FALL THERE HAS BEEN NO PERSONNEL TRAVELING NEAR THE FALL DURING THIS TIME
