EE was operating a 777 haul truck when a bottle of water fell off the buddy seat onto the floor. The ee reached behind the seat for the water bottle when his middle finger on his right hand contacted the partially guarded air conditioner fan blades. The ee received a chipped bone in the finger and a laceration that required sutures to close.

Employee was helping install a D10R center equalizer bar pin along with two other field technicians. The pin was struck awkwardly by another technician when a splinter of metal came off of the side and struck employee in the groin area of his leg.
While striking idler roller with hammer, a piece of steel broke off striking employee in forearm.
EMPLOYEE WAS REACHING A HYDRAULIC HOSE TO ANOTHER EMPLOYEE WHEN HE SLIPPED AND FELL ABOUT 7 FEET ON HIS RIGHT WRIST.
ENDLOADER WAS COMING FROM COAL STOCKPILE BACK DOWN TO RAIL SIDING WITH LOADER BUK RAISED BLOCKING HIS VISION AND RAN INTO LOADED COAL TRUCK. TRUCK DRIVER SAID HE SEEN HIM COMIONG AND STARTED S OUNDING AIR HORN ON COAL TRUCK. LOADER NEVER STOPPED, SO COAL TRUCK DRIVER STOPPED AND LOADER KEPT COMING AND RAN INTO THE LOADED COAL TRUCK.
FELL ON UNLEVEL GROUND TWISTING KNEE.
JUMPED OFF LOADER LANDING ON RIGHT FOOT.
