Employee was getting out of a pickup to throw away trash and when their left foot contacted the ground, they felt a pain in their left knee. Medical attention was sought, and crutches and a knee brace were issued.

Employee was cutting through a metal wall on 902 dragline, during this process the employee flinched. This caused the grinder to jump, and the cut-off wheel came into contact with employee's finger. Although gloves were in use, the injury still required sutures.

Mechanic was performing a routine PM on 571 OB truck. EE was changing a fuel filter on the right side of the engine block. With arm extended and pushing on the filter wrench EE felt a pop in shoulder.

A supervisor was working on writing an employee review in the office. As another individual walked past the office, they heard a loud noise and went into the office to see what it was. They found the supervisor lying on the floor actively having a seizure. Sometime during the event the left ear and left head suffered lacerations that required stitches.

On October 9th, 2024, an employee was in the process of putting on a new poly end. During the process the employee noticed it was going on crooked. Employee reached down to adjust the end at the same time another employee was swinging a sledgehammer. The hammer hit the employee's right index finger, resulting in a fractured finger.

Employee was assisting another employee helping to put a clevis on a machine. The clevis rotated and the employee's finger was pinched between the machine and the clevis.

Employee was assisting to load a backhoe onto a low-boy trailer. After loading and chaining the backhoe was completed, employee dismounted the trailer and in doing so felt a pop in their left knee.

On March 7th, 2024, a dragline ground person was in the process of pushing in a ramp off the bench to access the pit floor for cleaning. During the process of pushing in the ramp, the dozer blade caught a rock or frost edge. The dozer turned sideways, overturned and then came to rest on its tracks. The employee was wearing the seatbelt at the time of the incident.

On October 17th, 2023, an employee was dismounting 510-190 dozer. As the employee was stepping off the draft arm, they slipped and fell backwards. As they fell the left foot became wedged between the arm and the track of the dozer causing their lower leg and ankle to twist. This resulted in a fracture to the tibia and fibula of the left lower leg.

On May 31st, 2023, an employee was working to remove the crowd brake on 935 electric shovel to replace broken bolts. When the final bolt was removed, the stored energy in the crowd brake caused the brake to rotate. When the brake rotated, the employees arm came into contact with it. This contact caused lacerations to the employees' arm, which resulted in stitches.

Employee was dismounting blade and slipped on the ice catching self with left hand, causing pain and swelling in the left wrist. Employee sought medical attention on 1/15/2022 where there was suspicion of a non-displaced possible fracture. Employee sought a second medical opinion on 1/25/2022 and the left wrist was immobilized with a cast.

Employee was maneuvering becket end of pendant lines into a spool for storage. Employee had fingers between cotter key keeper on the pin and becket end. When the becket end was positioned onto the spool, back of pin made contact with spool sliding pin away pinching finger.

Employee was performing a welding/grinding repair on a Dragline bucket. Later in the shift the employee's eye became irritated. The eye was flushed several times and nothing was identified in the eye. The next morning the eye was still irritated. An eye doctor identified a metal sliver in the eye. Proper PPE was being utilized when welding/grinding work was being performed.

Employee was replacing a fan drive on an OB truck. Employee went to reposition and EE's foot slipped off of a crossover support. This resulted in EE's shin contacting a clamp bolt causing a laceration that required stitches. Employee was utilizing fall protection at the time of this incident.

An employee was in the process of starting to line bore. During this time the employee was shaking a can of spray lubrication to prepare to lube the cut. During this action the employees hand encountered the boring head which then pinched EE's right thumb against the bearing plate. This caused a partial amputation of the right thumb, and a broken right ring finger.

Employee was trying to remove a water hose fitting with a hammer and chisel. EE ended up missing the chisel and hit left thumb with the hammer. Employee was wearing gloves.

Employee was working on rebuilding a planetary. The planetary had 3 gears that were presumed stuck, a pry bar was used to free the first 2 gears. On the 3rd gear the employee was showing the shop planner that the gear was stuck. EE gently wiggled the 3rd gear & unexpectedly it freed up causing EE's finger to pinch in the housing. EE was wearing a latex mechanic glove.

EE was performing inspection on truck to start afternoon shift. EE lost EE's footing, regained EE's balance before falling, but felt a pop/burn in EE's upper right thigh. EE finished EE's shift, but decided to get a medical opinion on Monday. EE had a hip flexor muscle strain, EE was told to rest/ice for two weeks. Freezing rain and blowing snow conditions at the time of incident.

Dragline dump rope caught in drag hitch. Groundman moved drag ropes to create slack and went to park tractor. Two maint. employees developed a plan to use the boom truck arm to release the rope. While releasing the rope they did not see groundman enter area unannounced from behind. Rope released and contacted groundmans face causing injury to nose, lip, upper maxillary and teeth.

A mechanical failure of a drive-line on a Kress coal hauler caused damage to oil lines which started a fire in the rear of the truck. The "One Call" was made for the 30 min. fire qualification. The truck operator was not exposed to any injury as EE exited the truck off the front and remained clear. Emergency procedures extinguished the fire with no injuries to any personnel.

An employee was fabricating a swing case stand while working on a welding table. During the process the employee attempted to flip the piece over to continue welding and in doing so the steel frame fell off the table. The frame landed on the employee's foot, fracturing the foot.

A shop mechanic was dismounting a track dozer in the shop. The mechanic's foot slipped off a deck step and when trying to catch self the mechanic's left arm got caught between the hood handrail and the hood. This caused a fracture of the upper arm. The dozer steps were clean, dry and in good condition, as the dozer was washed for maintenance.

Employee was performing weld repairs in the car body of a loading shovel. Employee was wearing safety glasses & face shield, but at some point during grinding/air arcing a particle contacted the employees right eye. The eye was flushed & employee finished the shift and went on days off. On 8/28/18 the eye was bothering so employee went to eye doctor who removed a small particle.

Employee was cutting tack welds loose with a grinder. The grinder kicked back and struck employee's left leg causing a laceration.

Employee was kneeling down changing a tire on a pickup in the shop, while doing so employee's legs fell asleep. Employee went to pull themselves up by grabbing the side of the pickup and while doing so employee said they felt a tear in the lower abdomen.

A miner was carrying an aluminum beam assisting with a coal handling facility repair. The miners foot slipped off the bottom step of a staircase which resulted in the employee catching balance, causing mild compression fractures to vertebrae in the back. The miner returned to work with no restrictions.

A grass fire was discovered burning in a remote area next to a substation. The fire was burning away from the substation and any active work areas. No miners were endangered and emergency personnel responded and put the fire out. Extremely dry conditions in the area has the fire danger very high.

EE was washing small backhoe in wash bay. EE tried to reposition for better cleaning of engine. EE had left hand on handrail and both feet on the track. EE slipped and lost grip. Realized step was not below EE and made contact with floor.

A welder was die grinding a weld on a dragline mast repair and a particle of metal got in his eye. The particle/irritation was not noticed until later that evening at home. Welder was examined the next day by an eye doctor who identified the particle and removed it.

Employee was stowing the safety cable for the box of an overburden truck. The employee had trouble getting the clevis onto the hook. When the cable was moved the clevis pinched the employees finger against the box. The employees finger was cut and finger tip was fractured. Stitches were required.

Employee was setting light plant for Reclamation operations. Employee had set all the outriggers and was moving to unhook hitch pin when his feet slid out. Snow had covered an ice patch causing the slip. Employees left leg (shin area) contacted the light plant hitch causing a cut. The cut required stitches.

Employee was pushing an oil drain cart and in doing so pinched his thumb between the cart handle and a router bit on a scraper bowl. Employee was looking in the direction of cart movement and not paying attention of the position his hand was in. The weight of the cart pinched the thumb which required stitches.

Filling floor cleaning detergent and it splashed on his left foot. EE did not report it until the following day. I met him at the clinic. They sterilized his foot and recommended to not work until healed because of chance of infection.

Employees were pulling center pin on dozer frame hard bar and the pulling ram could not break the pin loose. Employee decided to hit the pin with a hammer. When he did a piece broke off the pin and hit employee in left cheek. the employee thought he was using a soft head hammer, but it was not. A physician was required to remove the steel chip and insert seven stitches.

Employee was walking into the main building from employee parking lot prior to shift. Employee had one foot slip on snow/ice and tried to catch them self. Employee went down on opposite knee and then contacted nose on the ground. The parking lost had been cleaned and sanded prior to the incident.

A diesel mechanic was preparing to install a radiator in an OB truck. Was positioned at front of engine and noticed the hood in a questionable position. Pushed on the hood to check if stable. The hood shifted and moved towards mechanic. The mechanic reacted to the movement and turned to get off truck. Mechanic went off the front bumper and contracted hands with the floor.

Employee was walking up a sloped pond embankment and felt a pop and burning sensation in the calf of his right leg. Normal work activities were ongoing and employee was not running and did not trip or slip. Employee was scheduled for a CT scan which diagnosed a tear in a gastro muscle of the calf. Physical therapy/healing time was ordered and employee could not RTW for 4-6 weeks.

Employee was grinding out a weld area on the mast of the dragline. Employee was wearing all his personal protective equipment. Grinder trigger was not engaged but disc was still spinning when grinder slipped in employee's hand, disc contacted left index finger cutting tip. Finger required 4 stitches, employee returned to work.

Employee was grinding out an air arc cut for repair on a front end loader. Employee was wearing safety glasses and a face shield. The grinding particle found its way under both and contacted his right eye. The work area was fairly open, but had some potential for particles to deflect around. The employee was taken to eye doctor who removed the particle. Employee returned to work.

EE was washing the top cab of a flatbed pickup and was stepping off the toolbox back onto the bed of the pickup. Stepped onto a crow bar and twisted his left ankle. After further examination found out ee fractured a small bone in his foot.

Mechanic was sitting on top of dozer transmission area doing wiring maintenance work. When EE was exiting area he heard a "pop" in his knee. EE experienced pain in knee & could not support weight on knee. He had been receiving personal med. attention prior to the incident. The symptoms of "popping" in rt knee were being diagnosed & treated by his attending physician.

EE was operating scraper & when looking backwards ran over a rock in the cut. Speed was slow while loading, but tire slipping off rock caused EE discomfort in tailbone area. There were no mechanical issues w/seat or scraper. EE worked next shift on 19th, but couldn't return on 22nd due to discomfort. A dr's appt recommended time to heal, but no treatment was administered.

On 4/8 an EE stepped off the personnel lift of a loading shovel and felt discomfort in their knee. Area had just been bladed & was relatively flat with no holes or chunks. The EE continued to work several days, but knee was bothering. EE saw a dr who recommended an MRI. Scheduling took time & EE continued to work. The MRI showed torn meniscus. Worked until 4/29, out for surgery.
