EE WAS OPERATING THE SHEARER CUTTING FROM #6 TO +6. A PIECE OF COAL BURST FROM THE FACE & STRUCKHIM IN THE HEAD. HE WAS WEARING A AIRSTREAM HELMET WITH A FACE SHIELD DOWN AND SAFETY GLASSES. HE RECD A SMALL LACERATION
EE WAS REMOVING SECTION OF MONORAIL. HE HAD TO LIFT ONE END UP TO REMOVE THE SUPPORT CHAIN., WHEN HE UNHOOKED THE CHAIN, IT SWUNG OFF THE MONORAIL AND THE HOOK HIT HIM IN THE MOUTH BREAKING HI S TOOTH
OIL ON FLOOR, EE SLIPPED STRAINING HIS GROIN CAUSING A HERNIA.
A ROOF FALL OCCURRED AT THE NO. 25 X-CUT, #4 ENTRY AT THE 2-WEST AINS SECTION. THE FALL MEASURES APPROX 15' WX20'LX6'H. THE AREA HAD BEEN SUPPORTED WITH 6' RESIN BOLTS WITH STEEL MAT AND WIR E MESH.
EE WAS CARRYING SOME MOTOR OIL TO PUT IN A SHIELD HAULER. HE SLIPPED ON A SNOWPACKED AREA AND FELL TO THE GROUND. THIS RESULTED IN A FRACTURE TO HIS RIGHT LOWER LEG.
MECHANICS WERE PUTTING BELTS BACK ON THE MAIN FAN. EE HAD HIS HAND ON THE BELT AS THE PULLY WAS BEING ROTATED. HE GOT HIS THUMB BETWEEN THE PULLY AND THE BELT CAUSING A LACERATION REQURING STITCHES
WHILE SETTING UP FOR MOVE OF FEEDER-BREAKER- EE BENT OVER & PICKED UP THE NIP END OF THE CABLE &THREW IT ON HIS SHOULDER. HE FELT A SHARP PAIN IN HIS LOWER BACK AT THIS TIME.
CARRYING A SMALL HAD CYLINDER TO THE MANTRIP SLIPPED ON ICE THAT HAD FORMED DURING THE NIGHT THE CYLINDER SMASHED HIS FINGER AGAINST THE ICE RESULTING IN A FRACTURE TO THE 5TH PHALANGE ON THE RIGHT HAND.'
EE WAS HANDLING A TIRE FOR A MANTRIP WHEN HE FELT A POP, AND ACCOMPANYING PAIN IN HIS ABDOMEN. HE REPORTED TO HIS FOREMAN BUT DID NOT NOTIFY HIS EMPLOYER THAT HE WAS SEEKING MEDICAL ATTENTION.
EMP WAS OPERATING THE SHEAR. FACE BLEW OUT A PIECE OF COAL HITTING EMP ON TOP OF THE LEFT FOOT. EMP LEFT FOOT BRUISED.
WHILE PLACING CRIBS ONTO MUTT, FOOT SLIPPED OFF PIECE OF COAL AND ANKLE SNAPPED--LEFT ANKLE & FLAKE FRACTURE.
EE WAS DRILLING A 6 FOOT BOLT HOLE WHEN A HYDRAULIC HOSE CAME UNDONE ON THE HOSE WHIPPED THE HIS HAND. THIS RESULTED IN A LACERATION TO HIS RING FINGER ON HIS LEFT HAND.
CRIB HAULER WAS UNDER EMULSION LINES. EMP WAS HELPING GET IT OUT. FINGER GOT CAUGHT BETWEEN ROOFJACK AND EMULSION LINE.
SECTION OF ROOF APPROX 4'W, 12'L, FELL IN AN AREA IN THE MAIN RETURN.
TOP OF RIB ABOUT 18"X 10"X 6" FELL. HIT EMP IN THE BACK CAUSING A COMPRESSION FRACTURE.
EE GOT HIT IN THE TEETH WITH A SOCKET WRENCH. THE WRENCH SLIPPED WHILE EE WAS TIGHTENING A BOLT ON THE BELT STRUCTURE.
EMPLOYEE HAD BEEN SEEING A CHIROPRACTOR FOR A NON-WORK RELATED PROBLEM. CAME TO US ON 8-5-96 AND ALLEGED AS WORK RELATED INJURY. WORKER'S COMPENSATION INVESTIGATED THE MEDICAL RECORDS AND ON 1 0-22-96 THEY ACCEPTED IT AS A WORK RELATED CLAIM MAKING THIS INJURY REPORTABLE.
CUTTING A PIECE OF CARDBOARD. CUTTING TOWARDS HIMSELF. KNIFE SLIPPED AND STRUCK HIM IN THE RIGHTFOREARM. RIGHT FOREARM, MAKING ONE INCH LACERATION REQUIRING 2 STITCHES.
THE LONGWALL SECTION EXPERIENCED A SEVERE BOUNCEAT APPROX 6:30AM. THE SHOCK WAVE BROKE SOME BOLTS AND ROCK FELL ON THE STAGELOADER. THE SECTION WAS SHUT DOWN AND REHABILITATION WORK STARTED.
ROOF AT CROSS CUT 34 #2 ENTRY IN 9 LEFT FAILED. THE FALL IS ABOUT 12' HIGH AND RIB TO RIB IN A 4WAY INTERSECTION. MEASURES ABOUT 20'X 20'X 12'.
EE WAS CARRYING TRUSS BARS WHEN HE FELT A SHARP PAIN IN HIS RT SHOULDER. THE EE DID NOT SEEK MEDICAL ATTENTION UNTIL 9-13-96.
WEEKLY EXAMINER DISCOVERED A ROOF FALL NEAR THE SEALS IN ONE NORTH BETWEEN 1ST AND 2ND RIGHT. THE FALL MEASURES APPROX 16' WIDE, 30' LONG, AND 8' HIGH.
EMP WAS DRIVING A SHUTTLE CAR AND HIT A HOLE IN THE FLOOR JARRING HIS LOWER BACK AND NECK. WAS ON HIS FIRST TRIP. WAS WEARING A SEAT BELT. DID NOT SEEK MEDICL ATTENTION UNTIL 7-29-96.
THE DAY SHIFT FOREMAN DISCOVERED THAT SOME MATERIAL HAD FALLEN BETWEEN THE SHIELD TIPS AND THE TAILGATE CRIBES, MAKING TRAVEL THROUGH TAILGATE QUESTIONABLE. AN INVESTIGATION WAS MADE BY THE SA FETY SUPERVISOR AND AN INSPECTOR THAT CONFIRMED THE BLOCKAGE.
EXAMINER FOUND THIS FALL AT 1600. THE AREA HAD BEEN EXAMINED 24 HRS. EARLIER AND THERE WAS NO ROOF FALL. THE FALL IS LOCATED AT #29 XCUT OF THE 1LHE SECTION. THE FALL IS APPROX. 28' WIDE, 10' HIGH AND 50' LONG.
EE HAD COMPLETED REPAIRS ON THE FEEDER BREAKER AND HAD WASHED OFF THE MACHINE. HE STEPPED ONTO IT TO PICK UP A WOODEN WEDGE. HE SLIPPED TO THE GROUND, TWISTING HIS ANKLE.
INTERSECTION AT X CUT 52 #2 ENTRY FAILED. IT HAD BEEN BOLTED WITH 6 FOOT RESIN BOLTS WITH STEEL MATS. HAD BEEN SUPPLIMENTER WITH 10 FOOT CABLE BOLTS AND TIMBERS ON THE CORNER IT FELL RIB TO RI B ALL 4 DIRECTIONS AND FELL APPROX. 11 FT HIGH.
ROOF FALL OCCURRED AT X CUT 31 PLUS 180 FEET IN 1 ENTRY SHEAR TRACTURE FROM AN ADJOINING FAULT CAUSED SOME BOLTS TO BREAK. THE ROOF SAGGED DOWN TO THE BOLT. IT DID NOT GO ABOVE THE BOLTS BUT D ID INTERRUPT PRODUCTION.
EMPLOYEE WAS SHCEDULED TO X-CUT 100 AND REPLACE A ROOF BOLT. HE HAD GONE IN AND PRE-SHIFTED THEN WHEN HE WAS JOINED BY ANOTHER EMPLOYEE THEY BEGAN TO WORK. A LARGE RIB 2'X20'X8' SLID INTO HI M. THE RIB PULLED OUT 1 FALL SHEET OF RIB WASH. IT STRUCK HIM ON THE LEFT LEG.
DURING A AAA INSPECTION, AN MSHA INSPECTOR AND UTAH FUEL CO OFFICIAL DISCOVERED A ROOF FALL AT XCUT 104 - 105 IN THE 9 LEFT BLEEDER ENTRY. IT ISUNKNOWN WHEN THE FALL OCCURRED. THE BLEEDER HAD BEEN EXAMINED ON 5-9-96 AND NO HAZARDS WERE OBSERVED. THE FALL MEASURES 18' X 20' X 6' HIGH.
BUS DRIVER HAD CLEANED THE STEPS AND DOGS AREA ON HIS PERSONAL BUS WHEN HE STARTED DOWN THE STEPS HE MISSED THE FIRST STEP TRIED TO GROB FOR THE HEARD RAIL AND MISSED. HE FELL OUT OF THE BUS O NTO THE GROUND.
EMPLOYEE WAS WALKING ON SURFACE ROADWAY HE SLIPPED AND FELL ON SOME SNOW AND ICE. HE DID NOT SEEK MEDICAL ATTENTION UNTIL 3-18-96 AND THE RESULTS OF THE EXAMINATION WAS NEGATIVE. DISCOMFORT DID NTO GO AWAY SO HE RETURNED TO THE DR ON 5-13-96 AT WHICH TIME THE DR PLACED HIM ON RESTRICTED DUTY AND THE INJURY WAS REPORTABLE.
ON FRIDAY MAR. 8 A ROOF FALL OCCURED IN THE 9 LEFT DEVELOPEMENT SECTION AT CROSSCUT 38. THE FALL WAS DISCOVERED BY THE SECTION MECHANIC. THE FALL ENCOMPASSES THE AREA FROM THE LEFT RIB OF ENTR Y NO. 2 & HALFWAY THROUGH THRU THE CROSSCUT BETWEEN ENTRIES 2&3. HIGHEST OF THE FALLS 15 FT. ABOVE ORIGINAL ROOF LINE. DIMENSIONS:90 FT X 20 FT X 30 FT. ROOF STRATA: INTERBEDED SHALES, SILT S
EE HAD JUST FINISHED LOADING 4 3C'S ON HIS FLAT BED TRAILER. AS HE WAS PUTTING THE NYLON STRAPS OVER THE 3C'S HE HAD HIS HAND BETWEEN TWO OF THE3C'S. ONE OF THE 3C'S ROLLED & CAUGHT HIS FINGER BETWEEN 2 OF THE 3C/S, LACERATING HIS LITTLE FINGER ON HIS LEFT HAND REQUIRING 3 STITCHES.
FIXING MINER TRYING TO SEE HYDRAULIC LEAK. HEAD IN TIGHT PLACE, LIGHT CORD CAUGHT ON A FITTING FUSHING HIS HEAD DOWN HITTING A PIECE OF STEEL.
HAD CLIMBED ONTO MIXER TO CLOSE THE LID. FOOR STRUCK LEVER AND STARTED THE MIXER BARREL TO ROLL,PINCHING EMP'S FINGER BETWEEN TWO PIECES OF METAL.
EMPLOYEE WAS WALKING UP THE BELT ENTRY. ANOTHER EMP STARTED THE BELT. THE BELT CAUSED THE FEEDER BREAKER TO ROLL FORWARD. INJURED EMPLOPYEE THOUGHT IT WOULD ROLL DOWN THE HILL TO HIM SO HE TRI ED TO RUN AWAY AND TWISTED ANKEL WHEN HE STEPPEDON A PIPE.
EE LOST HIS FOOTING & HIS PREVIOUSLY INJURD KNEEGAVE OUT CAUSING HIM TO FALL. HE LANDED ON THE PREVIOUSLY INJURED KNEE CAUSING MORE DAMAGE.
WAS CLEANING UP IN THE SHOP. PICKED UP PIECES OF METAL AND SMASHED FINGER BETWEEN 2 PIECES. DID NOT SEEK MEDICAL ATTENTION UNTIL 1-24-96.
HE WAS HELPING SPLICE A BELT. ANOTHER EMPLOYEE WAS USING A HAMMER TO INSTALL THE SPLICE. HE REACHED IN TO MOVE THE PUNCH AS THE OTHER EMPLOYEE WAS HITTING IT AGAIN. HIS RIGHT MIDDLE FINGER WAS HIT REQUIRING STITCHES.
EE WAS LOADING 8" CRIB BLOCKS ONTO A FLATBED. HE JERKED ON A CRIB AND FELT SOMETHING POP IN HIS ELBOW. HE REPORTED THE INCIDENT BUT DID NOT SEEK MEDICAL ATTENTION UNTIL 5-30-96.
EMPLOYEE HAD FINISHED WSHING #1 - #14 SHIELDS. AS HE WALKED BACK TO THE HEADGATE AREA HE SLIPPED OFF THE TOES OF #3 SHIELD AND TWISTED HIS ANKLEEMPLOYEE DID NOT SEEK ANY MED ATTN TIL 3-5-96
EE WAS DRIVING A COMPANY VEHICLE TO THE MINE FROM THE PRICE PARKING AREA. HE SWERVED TO MISS 2 DEER. LOST CONTROL OF THE VEHICLE & CRASHED INTO AN EMBANKMENT. HE WAS NOT WEARING A SEAT BELT AS REQUIRED & WAS EJECTED FROM THE VEHICLE THROUGH CLOSED WINDOW.
EE DID NOT REPORT ANY SPECIFIC INCIDENT. HIS CLAIM HAS BEEN INVESTIGATED BY WORKMENS COMP & DENIED ONCE. IT WAS RECIEVED A SECOND TILME & ACCEPTED AS A CUMULATIVE TRAUMA. IT WAS ACCEPTED ON 11 /30/95.
