THIS ACCIDENT WAS A LOST TIMEIN 1990 - THE PERSON HAS NOT TAKEN OFF AGAIN AFTER SUFFERING THROUGH SEVERAL YEARS OF BACK PAIN - FIRST DAY OFF THE JOB WAS 5/6/96. [ACCIDENT UPDATE]
THE DOUBLE BOOM BOLTER HAD BEEN SPOTTED IN PREPARATION TO START BOLTING. EMPLOYEE WAS STANDING BETWEEN THE LAST 2 ROWS OF ROOF SUPPORTING BOLTS WHEN A PIECE OF ROCK 4' X 4' X1.5" FELL BETWEEN THE BOLTS HITTING HIM ON THE LEFT SHOULDER AND BACK CAUSING A FRACTURE IN THE SHOULDER.
ROOF FALL OCCURRED ON 006-UNIT 5 BELTLINE X-CUT 14. LENGTH 28', WIDTH 17'5", 6-7'THICK.
GETTING OUT OF RAM CAR, BUMPED KNEE ON CAR FRAME- REPORTED AS FIRST AID ACCIDENT. UPDATED TO LOST TIME ON 5-13-96 ON FIRST DAY OFF JOB.
DUMPING 50 LB BAGS OF ROCK DUST INTO DUSTER HOPPER AND EXPERIENCED BACK PAIN. WENT TO CHIROPRACTOR FOR ADJUSTMENT.
BELT CREW WAS WORKING WHEN THE SPLICING MACHINE WAS THROWN ON THE BELT - BOUNCED OFF AND STRUCK EE ON THE KNEE. AFTER SEVERAL WEEKS HIS KNEE DID NOT HEAL AND HE DECIDED TO HAVE KNEE SURGERY- B ECOME A LOST TIME ON 3-29-96.
WHILE ROOF BOLTING, THE OPERATOR REACHED BACK TO PICK UP A ROOF BOLT OFF HIS MACHINE AND EXPERIENCED PAIN IN HIS UPPER RIGHT ARM. MUSCLE STRAIN.
EE WAS FUELING KCLOSENE HEATERS FROM A 5 GAL. FUEL CON AND STRAINED HIS SHOULDER AFTER CONTINUED DISCOUNTENT HE BECAME A LOST TIME ON 2 12 96.
ROOF BOLTING AND EXPERIENCED PAIN IN LEFT SHOULDER- WENT TO CHIROPRACTOR.
BOLTER OPERATOR WAS LOADING AND HIS MACHIN3E WITH BOLTING SUPPLIES HE WAS CARRYING A BOX OF GLUE FROM THE SUPPLY TROUBLE TO THE BOLTER. HE STEPPED ON A PIECE OF LOOSE KNOCK ON THE MINE FLOOR S LIPPED AND TWISTED HIS LEFT KNEE CAUSING A KNEE WROTE RX 2-6-96.
ROOF BOLTING WITH SECTION AUGERS--AUGER BROKE & ROTATION SWUNG IT AROUND & HIT OPERATOR ON THE INDEX FINGER OF THE RIGHT HAND. FINGER SET & HAND PUT IN CAST. OFF 3-4 WEEKS.
EE WAS DOING TIME STUDIES ON EQUIPMENT - SAT DOWN ALONG THE COAL RIB & THE RIB SHELLED OFF - STRIKING HIM ON THE HEAD, SHOULDERS, & BACK CAUSINGA LACERATION ON THE BACK BETWEEN THE SHOULDERS - POSSIBLE CRACK ON A SMALL BONE IN THE LEFT HAND.HE IS STILL WORKING.
WAS OPERATING A SHUTTLE CAR. WHEN MADE SWITCH FROM ONE SEAT TO THE OTHER, BUMPED KNEE. WENT SEVERAL WEEKS BUT AFTER CONTINUING PROBLEMS, BECAMEA LOST TIME ON 1-24-96 WITH KNEE SURGERY SCHEDUL ED FOR 2-1-96.
ELECTRIC MOTOR WAS BEING HOISTED BY A COME-A-LONG [HAND HOIST]. WHEN THE MOTOR CAME OFF THE FRAME IT SWUNG TOWARD THE HOIST OPERATOR. HE PUT OUTHIS HAND TO STABILIZE THE MOTOR & IT STRUCK HIM ON THE LEFT KNEE & CAUSED PAIN IN HIS RIGHT SHOULDER - AFTER SEVERAL WEEKS HE OPTED FOR AN OPERATION ON THE SHOULDER & BECAME A LOST TIME ON THEOPERATION DATE [2/27/96]
EE WAS ASSISTING IN THE ERECTION OF TRAP SETS ARCHS AND HIT HIS RIGHT HAND AGAINST A PIECE OF TIN HANGING FROM THE ROOF. THE MINE FOREMAN WAS LATE GETTING IN HIS ACCIDENT REPORT 12-15-95 AND W E WERE NOT PWORE OF THE ACCIDENT UNTIL DR BILLS ARRIVED AT THE MINE SITE.

INSTALLING A METAL LID ON #3 CCTT BOX AND EXPERIENCED PAIN IN LOWER BACK. AFTER SEVERAL WEEKS HE WAS DIAGNOSED WITH A BULGING DISC AND BECAME A LOST TIME ON 11-14-95. NEEDED BED REST.
PICKING UP SPARE OFF 3 WHEELER. STRAINED BACK MUSCLE. DID NOT GO TO DR UNTIL 10-3--95. GOT MUSCLE RELAXERS.
INSTALLING ROOF BOLTS PLACED PINNER AUGER AGAINST & APPLIED PRESSURE LOOSE GAB FELL STRICKING THE OPERATOR ON THE LEFT ARM JSUT BELOW THE ELBOW TOOK HIM TO HOSPITAL FOR XRAYS NOTHING BROKEN BU T HE HAD A SLIGHT REACTION TO PAIN KILLERS (ELEVATED BLOOD PRESSURE) (WAS WORKING AS TRAINEE ON DOUBLE BOOM BOLTER)

PULLING CABLE OFF REEL--STEPPED ON ROCK--TWISTED LEFT KNEE--DR. FINALLY DECIDED TO OPERATE AND THIS BECAME A LOST TIME INJURY.
CAR DRIVER GOT OFF CAR TO MOVE PINNER CABLE. LOOSE GOB FELL FROM THE ROOK HITTING HIM ON THE HEAD. AFTER SEVERAL WEEKS OF NECK PROBLEMS HE FINALLY BECAME A LOST TIME ON 1/10/96. BULDGING DISC IN NECK POSSIBLE OPERATION.
ROOF FALL ON UNIT #5-006 NO 5 ENTRY 2X-CUTS FROM FACE SPAD NO 17656 THICKNESS 5' WIDTH 24' LENGTH 36'
BELTMAN WAS THROWING BELT STRUCTURE UNDERNEATH A BELT LINE THEY WERE INSTALLING. HE HIT HIS ARM ON POINT OF THE STRUCTURE THAT WAS ALREADY INSTALLED CAUSING A CUT ON HIS ARM THAT REQUIRED STIT CHES.
INSTALLING 4 X 6 BARS-CAUGHT HIS FINGER BETWEEN THE X BAR & A ROOF BOLTER CANOPY FRAME-BROKE LITTLE FINGER ON LEFT HAND.
UNIT 1 HAD A BAD BATTERY ON THE 3 WHEELED UNIT WORK JEEP. THE MECHANIC WAS CUTTING OUT. THE BAD BATTERY & DROPPED THE LOOSE GROUND LEAD AGAINST A HOT LEAD. THE SPARKS CAUSED 2 BATTERIES TO BLOW UP, ACID WAS SPRAYED IN THE FACE & EYES OF THE MECHANIC. BOTH EYES WERE FLUSHED FOR 45 MIN. SLIGHT BURN IN RIGHT EYE NO PERMANENT DAMAGE.
EE WAS SETTING TIMBERS ON THE SUPPLY LOAD WHEN A SCOOP CAME THRU A NEAR BY CURTAIN AND STRUCK AND SUPPLY TRACTOR PULLING A MOTOR M THE WAGON BUMPED HIM. KNOCKING AND HIM TO THE GROUND. DID NOT GIVE TO THE DR UNTIL 8-31-95. FOUND NOTHING BUTOFTEN AND RETURN VISIT ON 9-8-95 THE DR RECOMMOUNDED 2 WKS LIGHT DUTY BECAME LOST TIME.
FLAL ON UNIT #1 NO. 2 ENTRY 2 CUTS OUTBY THE LAST OPEN X-CUT.
SAMPLING PERSON WAS PUSHING A LOADED WHEEL BARROW. WHEN HE DUMPED IT HE TWISTED HIS KNEE. CONTINUED WORKING FOR SEVERAL WEEKS THEN DECIDED TO HAVE KNEE OPERATION. BECAME LOST TIME ON 9-20-95.

FISHBACK WAS HELPING THE MINER OPERATOR WHEN A PIECE OF LOSOE GOB FELL. STRUCK HIM ON THE LEFT FOREARM CAUSIGN A LACERATION. HAD TO BE STITCHED.
NO PARTICULAR INCIDENT NOR TIME. EE REPORTED TAHT HIS SHOULDER STARTING HURTING & THAT IT WAS A WORK RELATED INJURY. HE FINALLY TOOK OFF ON. 1-5-96 AND IS NOW BEING REPORTED AS A LOST TIME . FACING POSSIBLE SHOULDER SURGERY. WITNESSES ARE TO TIME OF FIRST REPORTED PAIN ONLY.
EE WAS CROSSING OVER A STOPPED BELT. THE BELT STARTED, THROWING HIM TO THE GROUND AND BUISING HIS BACK AND LEG OTHER COMPLICATIONS FROM OUTSIDE SOURCES CAUSING LOST OF TIME JAIL MORITAL LTE CA USED A PROBLEM IN DETERMING IF THEN WAS LOST TIME DUE TO INJURY IT WAS DETERMINED AND LOST TIME ON 12-20-95.
ROOF BOLTER WAS PINNING AT THE FACE WHEN THE PINNER STEEL FELL BACK AND STRUCK THE OPERATORS LEFT HAND FRACTURED THE LEFT INDEX FINGER NEAR THE KNUCKLE PROABLY OFF 4 WEEKS.

ROOF FALL ON UNIT 3 MMU 007 NO 5 & 6 ENTRY SPAD 17196. LENGTH 140' WIDTH 18' THICKNESS 4'-5'.
EE WAS HELPING TO SPREAD ROCK DUST ALONG A BELTLINE AND STRAINED A MUSCLE IN HIS LOWER BACK WHILE LIFTING A 50 LB BAG OF ROCK DUST.
THE UNIT MECHANIC WAS REMOVING A BENT FLIGHT IN THE SHUTTLE CAR & ACCIDENTLY TOUCHED THE FLOME OF THE TORCH AGAINST THE CAR FRAME. THIS CAUSED SEVERAL PIECES OF RUST TO FLY OFF. THE MECH. WAS STRUCK IN THE LEFT EYE BY ONE OF THESE FLYING PARTICLES & HAD TO HAVE IT REMOVED BY DR. MISSED 1 OBSERVATION DAY THEN BACK TO WORK.
EMPLOYEE WAS BOLTING UP THE ROOF WHEN A PIECE OF ROCK FELL OUT BETWEEN THE #1 PIN & THE RIB STRIKING HIM ON THE ARM. THE ARM WAS CUT ABOUT 2-3" ABOVE THE WRIST & TO THE BONE. THE STITCHES WERE FROM THE BONE OUT (ABOUT 60 STITCHES) & DREDGING (CORRECTION: DRAINING) TUBE INSERTED. POSSIBLY OFF 2-3 WEEKS.
EE BENT OVER TO WALK THROUGH A CURTAIN WHEN HE RAISED UP ON THE OTHER SIDE OF THE CURTAIN HE JAMMED HIS HEAD INTO A PIN BOARD. CHIROPRATIC TREATMENTS.
DIESEL MECHANIC WAS ATTEMPTING TO SPREAD A TAPERLOCK WITH A SCREWDRIVER. THE SCREWDRIVER SLIPPED OUT, STRIKING THE MECHANIC ON THE LIP. STITCHES WERE REQUIRED.
