Roof Fall inby spad #2727 (60'L x 20'W x 4'H). The section was moved back 5 crosscuts.
Employee was helping to install canopy on shuttlecar when he drop it on his foot.
While traveling in the second right area of the mine which is not active on 10/19/09, A foreman observed a roof fall adjacent to spad #448. The fall is approx. 45' long x 18' wide and 1-9' thick.No escapeways,ventilation,travelways were blocked. No injuries. Fall is now plotted on map.
While setting a head drive employee stood up and felt a pain in left knee.
EE was setting timbers, when his back popped. He went to Jenkins Hospital, gave him an excuse to RTW on 3/6/08. He came in on 3/7/08 with another excuse to remain off work.
Employee was holding a piece of metal which was to be welded to tail piece when it fell over. He didn'ts let go and it pulled a muscle in his shoulder.
Employee brought scoop to outside for supplies. When he started to get off scoop he caught his left ring finger, causing a laceration to tip of finger and nail. He went to Pike E.R. to get stitches.
EE was injured on 7/27/06. He returned to work on 8/9/06 on regular duty. He stated that his shoulder started hurting again. On 8/25/06.
A piece of rock fell striking victim on head. Victim went to hospital received x-rays. Doctors stated victim had bruised muscles in neck. Victim returned to work on regular duty on 8/25/06.
Changing tire on mantrip, while letting jack down, bar flew out & hit him in mouth, causing cut on jaw & chipped tooth.

Employee was hanging 2" plastic waterline and stated he felt pain in lower back.
EE was pulling on a roof bolt to straighten it & strained his back.
WHILE MAKING A SPLICE TO SHUTTLE CAR CABLE, A PIECE OF THE CRIMPING TOOL FLEW OFF AND BECAME LODGED IN EE S LEFT THUMB.
EE WAS CHANGING BATTERIES AND FELT PAIN IN GROIN AREA. HE CONTINUED TO WORK UNTIL 1-29-04 AND HAS NOT YET RETURNED TO WORK AT THIS TIME.
WHILE OPERATING ROOF BOLTER THE WRENCH CAME LOOSE STRIKING HIM OVER THE LEFT EYE. HE RECEIVED STITCHES AND RETURNED TO WORK.
EE WAS PULLING A PIECE OF DRAW ROCK AND IT FELL ON HIS UPPER BACK CAUSING A CONTUSION.
WHILE REPAIRING MAT CHAIN ON MINER A PIECE OF ROCK FELL FROM THE ROOF STRIKING HIM ON BACK OF NECK AND HEAD CAUSING CONTUSIONS.
WHILE WORKING ON SPEED REDUCER IT DROPPED ON RIGHT INDEX FINGER CAUSING A FRACTURE.
WHILE OPERATING SHUTTLE CAR HIT HOLE IN ROADWAY HE STARTED COMPLAINING OF BACK PAIN
WHILE PUSHING ON HIS MANTRIP HE SLIPPED AND BRUISED HIS RIBS.
WHILE DRILLING A HOLE TO INSTALL ROOF BOLT A PIECE OF DRAW ROCK FELL STRIKING HIS LEFT SHOULDER CAUSING A CONTUSION.
WHILE OPERATING ROOF BOLTER A PIECE OF DRAW ROCK FELL ON HIS HAND CAUSING A FRACTURE TO ONE FINGER.
EE WAS DRILLING A HOLE AND WHEN HE REACHED FOR HIS LEVER TO LET THE HEAD DOWN HE GOT THE WRONG ONE AND HIT THE BOOM SWING LEVER AND IT SUNG OUT CAUSING HIM TO TWIST HIS BACK. THE LEVERS HAD BE EN MOVED RECENTLY PER K.D.M.M. AND MSHA.
WHILE SCOOPING RIB A PIECE OF DRAWROCK FELL AND HIT EE ON THE HEAD. EE SAID HE WAS NOT GOING TO HOSPITAL & HE WORKED REST OF SHIFT. DID NOT SHOWUP OR CALL FOR A WEEK. DID NOT REPORT ACCIDENT TO EMPLOYER AND FOLLOW COMPANY POLICY, SUCH AS DRUG TESTING ETC. EE DID NOT COME BACK TO WORK EVEN WHEN RELEASED BY DR.
EE WAS HOLDING A PIN WHILE ANOTHER EE USED A HAMMER TO INSTALL CHAIN AND THE HAMMER GLANCED OFF AND HIT EE MIDDLE FINGER ON HIS RIGHT HAND BREAKING IT. EE WORKED NEXT TWO DAYS AND DID NOT SEEK DOCTORS CARE BUT ON 11-13-02 HE WENT TO DOCTOR FOUND BREAK.
PULLED A PIECE OF DRAWROCK AND IT FELL AND STRUCK HIM ON LEFT SHOULDER.
WHEN BOLTING TOP FELT PAIN IN LOWER BACK.
WHILE BOLTING, EMPLOYEE MOVED ROOF BOLTER HEAD OUT AND CAUGHT HIS FINGER ON HIS LEFT HAND BETWEEN MINE ROOF AND BOLTER CAUSING LACERATION.
WHILE ON LIGHT DUTY FOR PREVIOUS INJURY HE WAS WALKING ACROSS MINE SITE, TRIPPED AND FELL. ON 10-17-02 WENT FOR CHECKUP FOR PREVIOUS INJURY AND DISCOVERED BROKEN WRIST. NO LOST TIME.
