Pulling a bearing off feeder shaft because bearing was down while pulling it off chain, slipped off bearing, striking them on the right wrist.
Roof had a slickened side to it plus it fell above anchorage.

THE EDGE OF THE UNIT STRUCK HIS RIGHT FOOT BEHIND THE HARD TOE RESULTING IN 4 FRACTURED TOES. ON 9/17/00 THE FOURTH TOE AND THE SMALL TOE WERE AMPUTATED DUE TO THE INJURY.
