EES HEAD WAS CAUGHT BETWEEN PICK/BREAK ER ASSEMBLY & R2OF OF MINE. EE WAS GIVEN 1ST AID BY AN E.M.T. & TRANSPORTED TO THE HOSPITAL. EE DIED 3-25-87 FROM INJURIES RECEIVED 3-16-87.
