EE was removing a tooth from a dragline bucket when a clump of dirt dislodged from the tooth socket and struck him in the face. Despite wearing goggles, a piece of dirt or rust apparently got in his eye causing irritation. EE decided to report the injury on 7/25/11 and lost 1 day of work after a foreign body was removed from his eye by a doctor on 7/25/11.
EMPLOYEE WAS WALKING TO JOB SITE AT 6:00 AM IN THE DARK TO HIS WORK SITE. EE STUMBLED ON A FOREIGN OBJECT CAUSING BACK PAIN.
EE WAS WORKING INSIDE THE HOUSE OF A DRAGLINE SHOVEL (REPAIRING COUNTERSHAFT) WHEN A GUST OF WIND BLEW A REAR DOOR PANEL OUT OF ITS TRACK AND IT STRUCK EE'S HEAD.
