Service Tech. was atempting to climb up on a dozer, as he grabbed the hand rail his hand slipped off the rail, resulting in the employee slipping backward and falling on the ground landing on his back.

Stepped out of truck and took a few steps and employees feet flew out from under him, resulting in hitting his head and knocking him out for a brief period.

Employee was walking up the haul road and slipped on the icy road resulting in a right arm fracture.

Employee was decending down the ladder of his loader. When he approached the bottom step of the ladder his feet slipped off the step, resulting in him losing his balance and landing on his back.

Employee was decending down the side of the D11R Dozer, as he stepped down on the pusharm his right leg slipped causing him to fall to the ground, resulting in pain to his right knee.

Employee was ascending up the ladder of his loader, when he slipped due to wet conditions from the coal pit, causing employee to hurt his right arm.

Employee was grinding on a piece of metal. While doing so a small piece of metal shaving flew through the side of employee's safety glasses, resulting in the metal shaving embedding in his right eye.

Employee was lifting metal out of the bed of his pick - up truck, as he was shifting his weight to throw the metal from the truck he felt a sharp pain run up his left side.

Employee was mounting up the ladder of a 992C loader to service the transmission fluid. As he approched the top step the oil hose reel unlocked, causing the hose to jerk the employee, resulting in him loosing his balance and falling off the loader.

Employee was prying on bar it slipped hitting the employee in the left eye lid resulting in him getting four (4) sticthes in his left eye lid.

Employee was climbing down ladder of 988F loader and as he approached the ground he twisted his foot and slipped on a block of coal causing him to fall to the ground landing on a block of coal resulting in a laceration of 1-1/2 " to his left thumb.

Employee was unloading a water pump from the bed of the F350 truck. The truck bed was slick due to rainy weather, therefore as he was descending the truck bed with one foot out and one foot in, he slipped on the wet surface falling to the ground. Twisted his right ankle, resulting in it swelling.

Employee was welding on a 777C Rock Truck Bed, and a piece of Slag flew up and struck the employee below the left eye, causing a mild burn below his eye.

Employee was backing up the dump with a 777C Rock Truck and stated that he either blacked out or fell asleep and over traveled the dump resulting in the truck over turning.

Employee was climbing down the ladder of the 777C rock truck and as he approached the last step, it swung under the bottom of the truck resulting in the employee falling to the ground landing on his right side.

Employee was tighting roller bolt on D11R dozer, Employee was lying on his back while tighting bolt and socket spun off head of bolt causing metal shavings to fly in employee eye, Employee did have safety glasses on

D9R dozer operator was traveling in reverse and struck a 777C rock truck in the rear, the rock truck was also traveling in reverse. The D9R operator states he didn't see the rock truck as he began to go in reverse, the rock truck driver also states he did not see the D9R dozer through his mirrors as he was traveling in reverse.

Employee was taking out top pin of 992C Loader boom, (Boom Pin) while pushing up on the pin employee felt sharp pain in his lower left side below his rib cage, causing him to pull a muscle.

Employee was walking from auger and while doing so he twisted his right ankle on unlevel pit floor.

While climbing up ladder employees left foot slipped off first step of ladder

Employee was lifting 45lbs wet hole bag from truck and was placing it into the drill hole (blast hole) Doing so Employees back began to hurt.
