FALL OCCURRED IN #5 HDG JUST INBY SPAD 414. THIS SECTION OF TOP HAD FALLEN OUT BEFORE TO A HEIGHT OF TEN FEET ABOVE COAL SEAM. WE HAD TOP SUPPORTED WITH STEEL ARCHES AND CRIBS. APPARENTLY A SE CTION OF ROCK BROKE OFF ALONG RIB, DILODGING CRIBS ALLOWING MORE ROCK TO FALL FROM TOP KNOCKING DOWN SEVERAL OF THE STEEL ARCHES.

FALL OCCURRED IN #5 HDG JUST OUTBY SPAD 45. TOP WAS BAD WHEN ORIGONALLY MINED AND HAD BROKEN BARS SET TO SUPPLEMENT BOLTS. BAD HAD BECOME ROTTEN AND ALLOWED TOP TO SAG. WE REMOVED SUPPORTS ALL OWING TOP TO FALL SO THAT IT COULD BE PROPERLY CLEANED UP AND REBOLTED.
