Showing all 18Employee was walking down road from drill bench to service truck and slipped causing a sprain to his right knee. On the 26th didn't report to work & contacted doctor. Reported back to work on the 27th. Went to emergency room on 28th due to pain and swelling.
Contractor was dismounting Cat D-11 dozer. As he got down to the lift arm, he slipped, fell backwards, struck the ground, injuring back and right wrist, sustaining a fractured thoracic vertebrae and right wrist.
Employee was working on dissassembled bed of a rock truck that was on the ground. He stepped off the bed to the ground, stepped on a rock and twisted his ankle. First aid was applied and he was transported to the hospital. He was diagnosed as having a chip fracture of his right ankle.
Driver loaded our Fusion Machine onto Pick-up truck, strapped unit down and stepped off back tire into hole in surface breaking right ankle
Employee was checking the oil on his Cat 980-F loader at the end of the work day. When he closed the access door a piece of dirt flew up and lodged underneath the eye lid. He worked the next day and went to hospital ER after work and had the foreign object removed. No lost time.
Employee was loading his 2007 Mack fuel truck with diesel fuel at the diesel fuel station. He stepped in the cab on the passenger side to warm his hands. As he got out of the vehicle, he slipped on a step, suffering a compound fracture to his right leg.
While moving a Cat. D11 single flange roller onto a pallet, the middle finger of the right hand was pinched between the roller shaft and another roller causing a 3 cm laceration in the finger. Employee received 9 stitches and returned to work following the medical treatment.
EMPLOYEE WAS LIFTING A TABLE BUSHING OUT OF THE MECHANIC'S TRUCK. BUSHING SLIPPED & CAUGHT HIS FINGER BETWEEN THE BUSHING & THE TRUCK BUMPER.

On top of drill removing a stand (cover), lost his balance. He tried to straighten up, but feet got entangled.
