THE SLATE TOP HAS CHANGED TO SANDSTONE FALLS USUALLY OCCURE UNDER THIS CONITION. A ROAD TO AND FROM THE SECTION WAS BLOCKED ONE OF THE THREE INTAKE AIR WAGE WAS BLOCKED.

INJURED EMPLOYEE REMOVED A HYDRAULIC HOSE FITTING FROM THE BOOM LIFT JACK ON THE 21 SC SHUTTLE CAR (OFFSIDE). THE BOOM FELL ON EMPLOYEE\S LEGS. EMPLOYEE FAILED TO INSTALL A SAFETY BLOCK UNDER THE BOOM.
