EE SAID A ROCK HAD LODGED WHERE #3 CONVEYOR BELTDUMPS ONTO #2 CONVEYOR. EE TURNED BOTH #2 & #3 CONVERYOR BELTS OFF AND WAS IN PROCESS OF REMOVING THE ROCK AND ACCIDENTLY GOT HIS BACK AGAINST THE OFF & ON SWITCH STARTING THE BELT AND DRUG HIM UNDER A WOODEN CROSSOVER USED TO CROSS THE BELT.

EE WAS TAKING OUT THE BREATHER PLUG ON THE RIPPER HEAD OF THE CONTINUOUS MINER WITHA CRESCENT WRENCH AND THE WRENCH SLIPPED OFF AND HE HIT HIS ELBOW ON A MINER BIT LUG.
A SHALEY HIGH WAS SAT DOWN INBY FAN HOUSING & BLOCKED OFF THE INTAKE AIRWAY. THE WHOLE SHALE PIECE MEASURED 7HX5WX2THICK. NO DAMAGE WAS DONE.
EMP WAS HOLDING A LINE UP OUT OF THE SCOOPS WAY.AS SCOOP CAME BY THE SCOOP OPERATOR SLIGHTLY MOVED THE SCOOP BUCKET AND CAUGHT THE EMP AGAINST THE RIB ON HIS THIGHS.
EMPLOYEE SAID THAT HE DIDN'T KNOW FOR SURE HOW HE DID INJURE HIMSELF. HE STATED THAT ALL HE DID WAS LOAD ROOF BOLTER SUPPLIES AND BUILD A STOPPING.
THE EMPLOYEE WAS DOWN ON HIS KNEES HELPING RUN A CENTER LINE UP ON WHEN HE GOT UP HE TWISTED HIS KNEE
EE WAS WORKING ON THE CONTINOUS MINER AND LIFTED THE RUBRAIL BACK ONTO THE MINER AND STRAINED HIS BACK.
THE ROOF BOLTER OPER HAD MOVED THE BOOM OF THE ROOF BOLTER ALL THE WAY OVER TO THE OPER SIDE OF THE MACHINE & STARTED BOLTING HE HAD INSTALLED 4OR 5 ROOF BOLTS WHEN HE HAD PLACED HIS HAND ON T HE SIDE OF THE BOLTER HEAD WHILE HE WAS DRILLING& CAUGHT IT BETWEEN THE HEAD & THE ATRS OF THE ROOF BOLT MACHINE
THE ROOF HAD 5 FT SOME 6 FT BOLTS IN IT NO CRACKS HAD BEEN DETECTED BUT SOFT AREA HAD BEEN THAT THE REASON FOR 6 FT BOLTS AFTER THE ROOF FELL YOU COULD SEE THAT THE ROCK HAD WET SLICK SIDES TO IT AND IT ALL BROKE UP INTO SMALL PIECES IT WASNOT A SOLID FULL. IT MEASURED 20 FT X 25 FT X 6 FT.
THE EMPLOYEE WAS CHANGING A SCOOP TIRE & THE TIRE FELL OVER ON HIS LEG INJURING HIS KNEE.
THE EMPLOYEE HAD STRAINED HIS BACK BEFORE AND WAS HAVING TROUBLE GETTING AROUND AND ON THIS DAY HE WAS MAKING A SURFACE ELECTRICAL INSPECTION AND WE WERE CHANGING A BELT ROLLER OUT ON THE BELT WHEN HE CAME OVER AND JOINED US. THE NEXT THING I KNEW I SAW HIM LIFTING AT A SPEED REDUCER AND I MADE HIM STOP LIFTING BUT HE HAD ALREADY DONE MORE DAMAGE TO HIS BACK.
THE EMPLOYEE WAS RETURNING TO THE SURFACE WITH THE MANTRIP AND STOPPED TO CLOSE A SET OF AIRLOCK DOORS ON THE TRACK AT NO 3 BELT HEAD AND WHEN HE GOT OFF TO SHUT THE DOORS HE EITHER FORGOT TO SET THE BREAK OR DIDN'T SET IT PROPERLY AND THE MANTRIP ROLLED INTO HIM PINNING HIM AGAINST THE DOORS. THE MANTRIP BRAKES WERE INSPECTED AND NOTHING WAS FOUND WRONG WITH THE BRAKE SYSTEM.
THE EMPLOYEE SAID THAT HE TRIPPED OVER THE MINER CABLE & FELL OVER ON THE CONTINOUS MINER BOOM.
THE INJURED WAS LAYING TRACK AND GOT HIS FINGER CAUGHT BETWEEN THE RAIL AND SCOOP BUCKET.
ROOF FALL OCCURED IN THE #4 ENTRY AS A CROSSCUT WAS TURNED TO THE RIGHT. FALL MEASURED 18' W, 20'L & 7' H. UPON INVESTIGATION OF THE FALL AREA, WE FOUND CUTTERS & SLIPS IN THE ROOF WHICH WERE NOT VISABLE IN THE HEAD COAL & THE ROOK BOLTS HAD RESIN ALL THE WAY DOWN THE BOLTS WHICH WOULD HAVE INDICATD TO ROOF BOLTER OPER, THAT NO CRACKS WERE PRESENT.
THE EMPLOYE EWAS HELPIN GON A ROOF BOLTER AND WATCHING TH EROOF FOR THE BOLT OPERATOR AND A PIECE OF ROCK FELL OUT BETWEEN THE ROOF BOLTS AND KNOCKED HIS HEAD INTO A TRAY ON THE ROOF BOLTEING MACHINE CUTTIN GHIS HEAD. HE HAD TO HAVE 4 STITCHES PUT IN IT.
EMPLOYEE STATED THAT REACHED ACROSS HIS BODT WITH HIS LEFT HAND TO PICK UP SOMETHING & INJURED HIS BACK THE DAY THAT I TALKED TO HIM HE TOLD ME THAT THE DOC T OLD HIM THAT IT WAS JUST A MATTER OF TIME BEFORE SOMETHING LIKE THAT HAPPENED BECAUSE OF LOWER BACK DEGENERATION
THE ROOF WAS SOFT IN THIS AREA AND WATER WAS DRIPPING IN THIS AREA. WE HAD NARROWED THE PLACES DOWN AND USED LONGER BOLTS BUT FROM LOOKING AT THE FALL THE ROCK HAD DAMAGE LAMINTIONS THAT WOULD NOT HAVE BEEN DETECTED BY THE ROOF BOLTS AND THATS WHAT HAPPEN IT JUST GAVE WAY. THE FALL WAS APPROX. 35FTL X 25FT W AND 8 TO 10 FT THICK.
THE EMPLOYEE WAS CARRING ROCKDUST TO ROCKDUST THE AIRLOCK ON NO. 1 BELT LINE AND SAID HE STAINEED HIS BACK.
HE ACCIDENTLY KNOCKED A POST WITH THE SCOOP AND A IECE OF ROCK CAME DOWN AND CAUGHT HIM AGAINST THE SCOOP. WHEN ACCIDEN FIRST HAPPENED HE DIDN'T MISS ANY WORK, ONLY COMPLAINED OF SOME STIFNES S. ON 10-19-93 I WAS INFORMED THAT THEY FOUND A BONE CHIP IN HIS HAND AND H E COMPLAINS OF HEADACHES.
HE WAS HLEPING PUSH A BUGGY PANNEL BOARD BACK IN AND STRAINE DHIS KNEE. THEEMPLOYEE SAID HE GOT HURT ON 10-6-93 BUT DIDN'T MISS ANY WORK UTNIL THE 8TH.
HE WAS HELPING INSTALL A SET OF TRACKS UNDER THE RAILRUNNER AND A MOTOR ROLLED OFF AND KNOCKED HEAD INTO THE SET OF TRACKS AND CUT HIS CHIN.
THE ROOF BOLTER OPERATOR WAS ROTATING THE STEEL AS HE BROUGHT IT OUT OF THE HOLE AND THE STEEL BROKE AND CAUGHT HIM ON THE CHIN.
A CROSSCENT WAS BEING STARTED FROM NO 6 TO 7 ENTRY THE CUT WAS ALMOST COMPLETED WHEN THE MINER APR. NOTICED THE ROOF WORKING HE BACKED UP AND THE NEW CUT FELL COMMING ALL THE WAY BACK INTO THE INTERSECTION THE FALL LOOKED LIKE A BIG SLIP THE ROOF IN TH9IS AREA WAS SHALE LIKE VERY GLOSSY LOOKING IN PARTS OF THE BALL THE FALL MEASURED APP 18FTW 25FT X 6 TO 10 FT HIGH.
ROOF FALL OCCURRED IN NO 3 ENTRY 1 CROSSCUT INBY PORTAL. IT WAS AN INTERSECTION FALL & NO EQUIPMENT OR PERSONS WERE INVOLVED. WE ARE STILL ABE TO TRAVEL IN AND OUT OF MINE. FALL IS ABOUT 18' W IDE & 20' LONG & ROOF HAD WATER DRIPPING OUT BUTHAS HAD FOR YEARS.
THE SCOOP OPR PARKED THE SCOOP & WAS G ETTING OUT OF IT & CAUGHT HOLD OF THE CANOPY TO PULL HIMSELFR OUT OF THE SCOOP DECK & TW ISTED HIS WRIST
THE EMPLOYEE STATED THAT HE WAS HELPING MOVE BELT AND FELT A CATCH IN HIS BACK BUT DIDN'T THINK ANYTHING ABOUT IT. AFTER GOING HOME AND THE NEXT DAY HE STATED THAT HIS BACK STARTED HURTING AN D HE WOULDN'T HARDLY WALK AND THAT IS WHEN HE WENT TO THE DOCTOR.
THE CONTINOUS MINER HAD TURNED A CROSSCUT TO THE LEFT AND BACKED OUT. THE EMPLOYEES HEARD THE ROOF CRACK A FEW TIMES AND IT FELL IN. THE FALL IS APPROX. 20-25' LONG, 17-18' WIDE AND 10-15' HIG H. THIS FALL WAS IN THE NO. 5 ENTRY 3 CROSSCUTS INBY THE TAILPIECE ON 7 RIGHT VERT. THIS FALL IS NOT GOING TO BE CLEANED UP BECAUSE WE ARE MOVING THE SECTION BACK TO START A NEW PANEL. THE TO
EMPLOYEE SAID THAT HE COULD NOT REMEMBER STRICKING HIS KNEE AT ANYTIME OR TWISTING IT HE SAID THAT ABOUT 1:30 HIS KNEE STARTED HURTING & HE COULDNOT HARDLY WALK ON IT
OPERATOR ALLEGEDLY HIT A HOLE WITH THE BUGGY AND INJURED HIS BACK. THE OPERATOR HAD JUST RETURNED TO WORK. HE HAD BEEN RELEASED FROM THE DOCTOR TO GO BACK TO WORK BUT TOLD ME THAT THE DOCTOR S AID FOR HIM NOT TO WORK ANY OVERTIME AND TO TRY OPERATING THE BUGGY.
A ROOF FALL OCCURRED ON THE 5TH RIGHT SECTION 001 IN THE NO.4 & 5 INTERSECTION AND CONNECTING CROSSCUTS. THE FALL IS APPROX. 120 FEET LONG 18 FT WIDE AND 10-15 FT HIGH. THIS FALL WAS DISCOVERE D ON THE MIDNIGHT SHIFT. THE FALL IS NOT GOING TO BE CLEANED UP BECAUSE THE PANNEL FINISHED ALL WE ARE GOING TO DO INCONNECT THE CROSSCUTS FOR VENT. THEN PULL OFF AND START A NEW PANNEL.
ROOF FALL STARTED 1 CROSSCUT OUTBY TAILPIECE IN NO 5 ENTRY ON RIGHT SECT & WENT FOR DISTANCE OFAAPPROX 10 TO 15' HIGH. SECTION IS BEING MOVED BACK NO FURTHER MINING IS PLANNED IN THIS AREA. VED BACK NO FURTHER MINING IS PLANNED IN AREA.
RUNNING A SHUTTLE CAR HAULING COAL TO FEEDER WHEN HE HIT HOLE IN PAVEMENT.BOUNCED HIM BACK AGAINST CANOPY LEG AND HURT HIS BACK.TOLD ABOUT THIS ACCIDENT WHEN EMPL WENT TO DR 11-3-90.

#5 INTUSECTION #5 TO 4 CROSS CUT AND #4 INTERSECTION FILL IN ABOUT 18' WIDE, 150' LONG 10' HIGH.
EMPLOYEE ALLEDGED THAT HE WAS SHOVELING BELT AND HURT HIS RIGHT HIP THIS ACCIDENT WAS NOT REPORTED TO ME UNTIL 9-13-90 HE WAS SHOVELING ON #3 BELT.
EMPLOYEE WAS LOADING BELT STRUCTURE IN SCOOP & T WISTED TO THROW A PIECE OF STRUCTURE & HIS RT KNEE GAVE WAY
LIFTING 6" HOLLOW BLOCK ? STARTED TO TURN HIS BODY AROUND AND HURT HIS BACK. (#20 LIFTING CINDER BLOCK).
THE EMPLOYEE THAT WAS INJURED WAS BENDING A ROOF BOLT. THE BOLT BROKE AND THE INJURED EMPLOYEE SPRAINED HIS LEFT ANKLE.
MAN WAS TAKING PART OFF BELT. THE BELT WAS SHUT OFF AND EMPLOYEE WAS CROSSING TO THE OTHER SIDE OF THE BELT. ANOTHER EMPLOYEE STARTED THE BELT AND THE INJURED EMPLOYEE JUMPED OFF THE BELT AND FELL INJURING HIS LEFT WRIST.
MAN WAS RIDING AND ARRIVING IN MAN TRIP. HE WAS SITTING ON HIS BELT SELF RESCUER. HE RAISED UP TO GET THE BELT SELF RESCUER OUT FROM UNDER HIM AND HIS HEAD HIT THE TOP OF THE ROOF.
INJURED EMPLOYEE WAS ON HIS KNEES AND WHEN HE STARTED TO RAISE UP HIS KNEE LOCKED UP. THE EMPLOYEE HAS HAD A HISTORY OF KNEE PROBLEMS.
HAD L. AHND LAYING ON ROCK SUTE ROCK FELL STRIKING L. THUMB.
BOLTER OPERATOR INSTALLED HIS FIRST ROW OF BOLTS AND STARTED MOVING HIS MACHINE AND STEPPED INTO A HOLE IN THE BOTTOM AND HURT HIS LEFT KNEE.
TOP BROKE ABOVE 5' ANCHORAGE FALL IS 75' LONG 18' WIDE 7' HIGH AT SPAD 7NW1650.
ROOF FALL AT SPAD 6NW IN THE #6 ENTRY 17 BREAKS FROM OUTSIDE 45' LONG 18' WIDE 5' HIGH.
