HE WAS COUPLING TWO PIECES OF BOLT DRILL STEEL TOGETHER, PUSHING LEVER TOO SOON THEREFORE PINNING HIS INDEX FINGER BETWEEN BOLTER STEEL AND MINE ROOF.
STEPPED FROM TRACK FLAT CAR ONTO MINE FLOOR. TURNING ANKLE.
A FAULT IN THE TOP CAUSED THIS FALL. THE FALL WAS 120' LONG AND 2' TO 4' THICK. THIS FALL FELL BETWEEN THE LAST TIME THE AREA WAS TRAVELED 7-21-88 TO 7-25-88. THIS FALL WAS IN THE RETURN HOWEV EVER, DID NOT AFFECT THE AIR ON TRAVEL OF MEN.

W VA ARMATURE JEEP OPERATED BY STEVE CANFIELD WAS COMING DOWN A HILL THE LEE NORSE JEEP WAS STOPPED AT THE BOTTOM OF THE SAME HILL. THEY WAS PUTTING THE TROLLEY POLE BACK ON THE WIRE. WHEN THE W VA ARMATURE JEEP SLIDE ABOUT 30 FT DOWN THE HILL HITTING THE LEE NORSE JEEP JARRING THE RIDERS INSIDE.
VICTIM WAS RIDING IN DECK OF BATTERY TRACTOR ANDWAS ROLLED OUT OF DECK OVER TOP OF TRACTOR.
WRENCH SLIPPED OFF BOLT HEAD AND STRUCK VICTIM ON THE SIDE OF THE HEAD.
VICIMT WAS PITCHING A FULL CAN OF OIL UP TO THE MINER.
OPERATOR'S SHIR7 SLEEVE CAUGHT UP LEVER CAUSING HEAD TO GO UP CATCHING FINGER BETWEEN HEAD AND MINE ROOF.
STARTING THE DR3LL STEEL KNEE HIT FAST UP DRESSING CAUSING DRILL HEAD TO MOVE UP CAUGHING LEFT THUMB.
