Showing all 47Company security guard was conducting a survey when he discovered a Mack DM 600 fuel truck on fire. MSHA was notified and division of mine safety. The local fire department was dispatched to the scene and extinguished the fire.
Employee was standing on the tracks of a Gen set went to step over and slipped and fell catching self with his left arm. Pulled muscle in shoulder.
EE was changing the bucket teeth on a 992 Loader while using a hammer to knock the teeth off a piece of metal flew from the tooth and hit him in the left side of his stomach.
Employee was helping install lift jack on 900 loader. The cable came off the pulley of boom truck giving slack to the chain hook. The hook hit employee on top of his head cutting it. He received 4 stitches. Came back to work next day.
Employee was operating fork lift placing beams on high wall miner table. When the high wall fell on fork lift he received a laceration to the head and a compound fracture to right arm.
Employee was moving a 55 gallon drum of antifreeze when the drum slipped and mashed his middle finger on left hand against another drum of antifreeze. He received 5 stitches on finger. Came back to work next day.
Employee was welding on a loader when he twisted to get to the work spot to weld he felt pain in his neck.
Employee was working on a drill when he bumped his head on the deck of the drill, cutting the top of his head. He received 3 stitches. No lost time.
Employee was climbing down from D-9 Dozer. When he stepped off of the dozer track onto ground he hurt his right ankle.
EE was using a torque wrench torquing head mounting bolts when he strained his lower back.
Employee was operating Dozer running the fill and said his back was hurting him.
Repetitive Motion
Employee was operating rock truck going up a hill. The truck spun out and slid backwards straining his back. Employee did not report it to his supervisor until 12/27/11 and went off work on 1/2/2012.
EE was getting on a excavator, he had one foot on excavator step and raised his other foot up to get on excavator and fell backwards on his back.
Employee was pulling on lever to let prell in the prell truck and his back started to hurt.
Employee was entering dozer when he grabbed the hand rail to pull himself onto top step when he felt his right shoulder pop.
EE was operating a rock truck when he was backing up to get loaded he backed into the loader.
EE was operating a rock truck coming out of the coal pit and the tire hit a hole in the road spinning the steering wheel and it jammed the EE's right shoulder.
EE was helping install rear end in truck. Felt pain in shoulder while lifting rear end. Continued to work full time until scheduled for surgery. Will be off work for 3-4 months for recovery.
Employee was priming holes when he stepped in a crack on the drill bench and twisted his right ankle.
Driver drove truck into ditch and over turned. He received 3 stitches on his left jaw.
Employee was hitting a sprocket with a hammer and a piece of hammer chipped off and hit him on his right lower arm. He received 6 stitches. No lost time

EE was loading holes, when he got back in power truck to sit down, his lower back started to hurt. He went off work on 6/22/09.
After maintenance was done, EE was gathering tools and when he bent over a slate bar was hanging out from the cat walk, EE ran into it. He got 3 stitches over his right eye. No lost time.
It had been raining all night and was now down to a sprinkle. It wasn't completely daylight due to the rain. EE was pulling a winch cable out of the boom to hook onto the dozer track. He was walking backwards in the mud and tripped over a sprocket segment with his left foot. He slipped and fell on his right hip.
Employee was getting off d-11 dozer when his foot stepped off of the step, When he landing on the ground. It hurt his right knee
Tramming drill with door open and hand on door facing. Door shut and smashed EE's fingers. Fingernail removed from middle finger on right hand.
Employee was helping change a packing kit on a power head sump cylinder he was talking the bolts out of the cylinder head. When the head blew off and hit him in the upper left leg. He received bruising on his upper leg.
EE was removing radiator hose on Cat 992 loader, when hot anitfreeze splashes on his left arm. Burned top of hand and arm.
EE was getting out of Prell Truck, when he slipped off step and pulled muscles in his back. EE did not go off work until 6/17/08, he came back on 6/23/08.
He was coming down ladder off of the highwall miner when his feet slipped causing him to fall and strike his right side on the base of the frame ladder. He fell 4' & fractured 1 rib on his right side.
He was tightening a bolt on the highwall miner. When the wrench slipped off bolt, he fell on his left wrist and fractured it.
Hi Wall fall, on SHM Miner #17. Approximate size of fall was 60' high, 20' wide and 125' long. No employees were involved. Employee was doing maintenance on miner at the time. Miner was backed away fron the Hi Wall at the time of the fall.
He was working on hi wall miner when he stepped off miner, raised up and hit his head on beam table. Cut on top of his head, got 7 staples. No lost time.
He heated a piece of metal on the blade of a dozer. Another employee struck it with a sledgehammer. The handle broke on the hammer. The head of the hammer went into the air and came down and hit employee on top of his head. Cut on top of his head. 6 staples, "{no lost time".
Getting down from highwall miner. When he stepped off ladder onto ground he twisted left ankle. It fractured top of left foot.
Climbing out of grader he slipped and fell from steps to ground about 3 to 3 1/2 feet. Landed on right side bruising ribs and shoulder.

Putting tie rod on rock truck. Tie rod slipped off hitting his pinky finger on right hand - 3 stitches. No lost time
Install pedistal in beam for hi wall miner and strained his back was not reported until l 5-05.
Employee was watering road at Mayo #5. Going up hill when engine bulged down. Truck started rolling backwards, hit berm and turned over. Bruised his neck/hip/& left leg. No Lost Time.
Working on hi-wall miner repairing piller box. Bent over using a hammer. Strained his back.
EE HAD REMOVED A BROKEN BOLT FROM THE UNDER CARRIAGE. HE WAS SLIDING OUT FROM UNDER THE UNDER CARRIAGE WHEN THE WELD BROKE AND ONE OF THE BOGGIES SWUNG DOWN HITTING HIM. IT BROKE HIS RIGHT AW AND BRUISED HIS RIGHT SHOULDER.
EE WAS CHECKING ANTIFREEZE IN D11 DOZER WHEN HE LOOSENED RADIATOR CAP, ANTIFREEZE CAME OUT AND BURNED LEFT HAND.
THE EE WAS BACKING LOADER UP TO GET BUCKET OF COAL & BACKED INTO HIGHWALL.
