As EE was returning to equipment after lunch, a gust of wind blew dust into his eye. EE rubbed his eye to remove dust particle, resulting in a scratch on surface of eyeball.
Loader blew a hose, leaked oil, causing a fire. The EE panicked and jumped off loader, injuring his knee.
Loader operator dropped bucket onto back dump hauler truck, jerking the hauler truck operator.

NO CONTRIBUTING FACTORS. EE SLIPPED EXITING VEHICLE AND FELL ON ARM.

OPERATOR DROVE HIS BACKDUMP TRUCK UPON THE FILL AREA AT LUNCH BREAK, AND IN THE PROCESS OF STEPPING DOWN THE LADDER HIS LEFT FOOT MISSED THE BOTTOM STEP ADN CAUSED THE OPERATOR TO TWIST TO ONE SIDE. THERE WAS NO PAIN OR DISCOMFORT UNTIL THE NEXT MORNING WHEN HE WAS GETTING OUT OF BED.
