Employee was tightening a bolt and had pain in left shoulder, notified office on 9/28/15. Went to emergency room on 9/26/15 with pain in shoulder and starting down his arm and they said he need to see an orthopedic doctor. Appointment on Wed 9/30/15.

EE was walking to his truck from dragline after his shift. The ground was uneven and snow covered.
PUTTING 6" BEARING IN BAR ON D-10. TAPPING IN THE BEARING CAME OFF AND WENT INTO RIGHT KNEE
EE WAS WORKING IN A SMALL SPACE BETWEEN ENGINE AND BATTERY BOX. EE WENT TO STEP UP AND TO THE RIGHT OF THE BATTERY BOX. THE RIGHT FOOT DIDN'T MOVE AND HE "STOOD UP I TWISTED MY KNEE"
WHILE GETTING OFF MACHINE (DRAGLINE) HOLDING ONTO A PAD ON TRACKS WITH RIGHT HAND, HIS RIGHT FOOT LANDED ON A ROCK THAT STARTED TO MOVE. KEPT HOLDING ON TO AVOID A FALL BACKWARDS. HIS SHOULDER BECAME DISLOCATED-THEN FELL AND LANDED ON HIS REAR.
