THE ROOF FALL IN A CONTINUATION OF THE FALL WHIX OCCURRED 08-09-89 AND INVOLVES 5+90-6+60 OF ENTRY #4 OF OUR MAIN EAST DEVELOPMENT.THE BOLTERS HAD MOVED THE MACHINE OUTBY THE 1ST FALL WERE CRI BS AND RAILS HAD BEEN INSTALLED.THE FALL RODE OVER THE RAILS.
THE ROOF FALL IS A CONTINUATION OF THE FALL REPORTED ON 08-07-89.AS REPORTED ON 08-07-89.AS REPORTED ON THAT FALL CRIBS AND RAILS WERE SET,BUT THE FALL RODE OVER THEM.THIS FALL OCCURRED AT 4+9 5 THRU 05+50 OF ENTRY #4 ON OUR MAIN EAST DEVELOPMENT-THE FALL PEAKED OUT AT APPROX 27'.
ROOF FALL OCCURRED FROM 6+55 OF ENTRY #4 OF MAIN EAST DEVELOPMENT INVOLVING 6+30 X-CUT. FALL WAS APPROX 6' HIGH.
THE ROOF FALL OCCURRED AT APPROX 5+35 OF ENTRY #4 ON THE MAIN EAST INVOLVING THE 5+70 X-CUT.ROOF FALL WAS APPROX 6' IN HEIGHT.
WHILE CUTTING PIECE OF WATER HOSE WITH UTILITY NIFE. KNIFE SLIPPED & CAUSED 1" LACERATION ON LEFT CALF. THIS ACCIDENT OCCURRED AT SURFACE LOCATION.
WAS UNLOADING RAIL BARS FROM SUPPLY CAR WHILE SWINGING RAIL OFF OF CAR. DID NOT RAISE END HIGH ENOUGH. CATCHING FINGER UNDER RAIL SMASHING & SPLITTING IT OPEN.
WAS WORKING ON CONTINUOUS MINER WHILE WALKING AROUND IT TWISTED ANKLE & FELL. WAS TEARING IT DOWN FOR SCRAP.
SIP IN FR WHICH RAN THROUGH THE 4+60 X CUT BETWEEN ENTRIES #3 + #4 RIAL CARS INSTALLED FOR ADDED SUPPORT OCC IN ENTY #4 UP TO 5+25 AND WAS A8' H IGH S + S SCOOP COVERED JUST IN FRONT OF OPER C NAOPY INCL BUCKET.
TAKING BOTTOM GUARDS OFF SLOPE BELT CORNER OF ONE OF GUARDS LACERATED BACK OF R HAND.
WAS LIFTING 6X6X6 TIMBERS OUT OF MUD PLACING THEM AGAINST RIB,IN ORDER TO GET THROUGH.
MINE SETTING IDLE, AND PREVIOUS OPERATOR GUTTING WIDE INTERSECTIONS. FALL IN ENTRY #5 FACE T-CU.
THE FALL OCCURRED AT THE 1+50 X-C OF ENTRY #3. THE PREVIOUS OPERATOR CUT WIDE INTERSECTIONS AND THIS MINE HAS BEEN SETTING IDLE SINCE 12-28-87. WE ARE VENTILATING MINE IN ITS PROPER MANNER.
WE FEEL THE MAIN CAUSE OF THE ROOF PROBLEMS ARE DUE TO THE WIDE INTERSECTIONS THAT WER CUT BY THE PREVIOUS OPERATOR AND THE MINE HAS BEEN SETTING IDLE. VENTILATION HAS BEEN MAINTAINED.
