INJURED EMPLOYEE WAS CLEANING COAL WITH HAND SHOVEL.ALTHOUGH WARNED ON 2 PREVIOUS OCCASIONS TO WEAR HIS HARD HAT,HE WAS NOT EWARING IT WHEN A SMALL PIECE OF SHALE FELL FROM SPOIL BANK AND STRU CK HIM ON HIS SCALP.15 STITCHES WERE REQUIRED TO CLOSE THE WOUND.
