ALL THREE FUSES OF INCOMING 12470 VAC BLEW AT APPROX 8:45 AM. ALL THREE FUSE HOLDERS WAS REMOVED OMPLETELY & REFUSED. EMPL WAS INSPECTING DEENERGIZED SUB STATION TO TRY TO DETERMINE PROBLEM. L IGHTING ARRESTER WAS DISCOVERED WITH HOLE IN IT.EMPL PRECEEDED TO INSTALL ONE FUSE HOLDER & ENERGIZED ONE PHASE. HE THEN CAME IN CONTACT WITH ENERGIZED PHASE.

EMPLOYEE WAS IN THE PROCESS OF PLACING A WOOD BLOCK UNDER A SECTION OF 8" STEEL BEAM WHEN HIS CO-WORKER WHO WAS LIFTING THE BEAM THROUGH HE HAD BLOCK IN PLACE HE LET GO OF THE LIFTING BAR, THU S DROPPING THE BEAM ON INJURED PERSON'S HAND.
