Employee was attempting to clean a return roller on conveyor under the HP 300 crusher, against company policy, without utilizing lock out tag out training. EE was using a bar to scrape mud off the return roller. The bar was caught between the roller and moving conveyor belt, pulling the EE's left arm into the pinch point and resulting in broken bones and a dislocation.

There were high winds at the site. Debris was blown into the left eye of employee. Employee did not have discomfort until around midnight 02/14/2023. Employee was wearing safety glasses at time.
