Showing all 46A piece of draw rock measuring 0"-2" X 24" X24" fell on canopy and slid off hitting subjects left leg and foot, fracturing left tibia about 6 inches above ankle.
Subject was trying to straighten roof bolt. Canopy struck subject in head. Stab jack was set on loose bottom. Bottom busted and canopy struck subject in head. Head aches. Waiting on test results.
Injured back (strained) while shoveling loose coal and putting on belt conveyor.
A non-injury roof fall accident has occurred in the No 1 entry of the return on the 1st North West Mains. The fall was discovered during the weekly exam of the return air course. The fall is located approximately 10 feet outby survey station No 2318. The fall is approximately 50 feet in length, 20 feet wide and 6 feet thick.
A non injury roof fall occurred in the #3 entry of the return on panel 4 off North East mains. Additional roof support had been installed in this area prior to the fall occurring.
A non-injury roof falls occurred in the out-by return air course. The roof falls were discovered during the weekly exam of the return air course. The falls was approximately 50 ft. long x 19 ft. wide x 6 ft. in thickness, and 12 ft. long x 12 ft. wide x 5 ft. in thickness.
Freezing and thawing temperature changes contributed to material becoming unstable. The effected area did not show any signs of weakness or deterioration prior to rock/material sliding off highwall into pit.

The continuous miner operator was tramming across the working section, tripped and fell causing him to lose control of the machine pinning him against the coal rib resulting in a fracture of the top & bottom of pelvis (left).

When driving mantrip out hit small rock with wheel making steering wheel hit his right hand. He went to hospital. After he went home he called back and said it was just bruised.

EE CLAIMS THAT RIB ROLL HIT HIS RIGHT HIP. BRUISED RIGHT HIP. WHILE OPERATING EIMCO CONT. MINER.REMOTE CONTROLLED.
EE WAS LIFTING CRIB BLOCKS. HE SAID HE FELT A PULL IN HIS LEFT ABDOMEN AREA. THIS ACCIDENT HAPPENED ON 2/22/03. HAD NO LOST TIME UNTIL HE WAS OPERATED ON 4/16/03 FOR DOUBLE HERNIA.
SUBJECT WAS TRAMMING DDO-13 FLETCHER ROOF BOLTER, CROSSING CONT MINER CABLE HE WAS TRAMMING IN REAR DECK WHEN HE CROSSED CABLE THREW REAR DECK WAS NOT IN LOW POSITION IN TO MINE ROOF. RIGHT UP PER ARM AND SHOULDER HIT ROOF.
SUBJECT WAS OPERATING 21SC SHUTTLE CAR ON 002 SECTION, RAN OVER A PIECE OF ROCK IN ROADWAY. ALLEGES THAT IT SHOOK HIM CAUSING PAIN TO BACK.
SUBJECT WAS OPERATING REMOTE CONTROLLED CONT/M WER PIECE OF DRAW ROCK FELL FROM ROOF HIT HIS RT HAND.
UNKNOWN, EE SAID HE DID NOT KNOW HOW OR WHEN HE HURT HIS BACK.
SUBJECT WAS BOLTING ROOF, AND A SMALL PIECE OF DRAW ROCK STRUCK HIM ON LEFT SHOULDER.
A PIECE OF DRAW ROCK FELL & SLID DOWN SHOULDER & NECK WHILE OPERATING THE REMOTE CONTROLLED 2480-1 EIMCO CONTINUOUS MINING MACHINE.
EE WAS HELPING TO MOVE THE CONTINUOUS MINER CABLE AND COAL RIB ROLLED OFF, STRIKING HIS BACK JUST ABOVE THE WAIST LINE.
SUBJECT WAS OPERATING ROOF BOLTING MACHINE, CAUGHT IS HAND BETWEEN THE PIECES OF PINNER STEEL.
