HE WAS TRAMMING TO FACE WHEN HE WENT OVER A LITTLE ROCK IN THE FLOOR MAKING THE RIDING COMPARTMENT RAISE CLOSER TO THE ROOF. HE HAD HIS HEAD LEANDED OVER TOWARD THE MACHINE AND HIS HEAD WAS CA UGHT BETWEEN ROOF AND MACHINE. HE PULLED HIS HEAD OUT AND INJURED HIS NECK DOING SO.

VICTIM WAS AT THE BACK OF ROOF BOLTER APPROXIMATELY 25' FROM FACE. HE WAS BENDING ENOUGH BOLTS TO BOLT THIS PLACE WHEN A PIECE OF ROCK 24'X24"X1" FELL FROM BETWEEN BOLTS HITTING ON SIDE OF HE AD. HE SUSTAINED 3 SMALL CUTS TO LEFT EAR.

SEVERAL FINE STREAKS OF COAL RAN UP INTO TOP TO ABOUT 40 TO 45 INCHES CAUSING THE ROOF TO BE WEAK IN THIS INTERSECTION. ROOF FELL IN THE INTERSECTION, NO EQUIPMENT OR PEOPLE WERE INVOLVED.
