The employee was flipping the lid of a grid box when they felt a sharp pain in upper left arm. They requested help from company EMTs and was then transported to hospital. The hospital released them that night and advised them to follow up with Primary Physician and placed them off work until 4/4/2026. The employees Primary put them off work until they are cleared from Orthopedic.

Showing 50 of 178The Employee hit a rock in the road while grading, bringing the grader to an abrupt stop. EE continued to work but complained of pain in EE's neck, legs, and feet. Went to the hospital the next day, had X-Rays and issued Physical Therapy. The extend of EE's condition is unknown at this time.
While removing access stairway from RT62, with the top of stairs attached to a boom, and the bottom welded. The employee forgot to remove the top two bolts, they went back up the stairway and when they removed the bolts, the weld at the bottom broke and the stairs shifted causing them to fall to the ground. The employee suffered a fracture on the radius of their right arm.
The Employee was going to turn off the water valve after filling up the tank on EE's steam truck. The employees foot slipped and EE caught self, felt a sharp pain in lower back. Went to the doctor a few days later x-rays shown nothing, given time off with meds.
Employee was walking on a drill bench and stepped into a drill hole felt a pop in right knee but continued to work. On 9/10/2025 the employee was taken off work by their doctor for torn Meniscus.
After climbing up on to the track of a Dozer, the employee was attempting to latch the drop-down handrail when the handle broke and they lost balance and fell. The employee was taken to clinic, employee has fractured Spinous Process, given medication and rest for a week.
The employee went to the hospital on 8/25/2025 suffering from numbness in left arm and leg. They told management that they were running tests for stroke and other possible issues. They stated getting jarred in an excavator on the 8/23/2025 but didn't feel it was related. Came back to work for three shifts and took off again for a strain in their neck and numbness in left arm.
Employee was tightening a pipe union. The wrench slipped off. The employee later stated that EE's elbow was hurting.
Employee was driving to the Coal pit and hit something in the road, EE was jarred and said EE's back and leg was hurting. Went to EE's Dr. and was given days off to rest.
Employee was exiting the Number 2 truck dump slipped on the step and fell. EE went to Logan medical center and x-rays show a fractured rib.
Employee was changing a starter on a Rock truck; they had the starter placed on a step ladder. The starter rolled off and landed on their left foot behind the steal toe. Employee went to the doctor had a bad bruise told to rest.
Employee was walking around equipment steam cleaning, stepped into a low place and twisted EE's ankle. Had x-ray and told to take off work for a sprain ankle.
Employee was operating an excavator above an embankment, when the excavator began sliding down the bank. EE tried to swing the boom around to stop the slide, but the excavator rolled onto its side. EE was taken to the hospital where the x-ray revealed a compound fracture of the T-12. EE was given meds and sent home; EE is awaiting an appointment with a neurologist.
Employee was working on a rock truck when a cordless drill fell from the top of the engine and landed on EE's right hand causing a fracture to the end of EE's finger. EE went to the ER and had a splint placed on EE's injured finger.
Employee was operating a tractor trailer coal truck when they hit a rough spot in the road. Lower Right back began hurting, EE was taken to PMC hospital. The employee was x-rayed, given medicine and taken off work for a strained back.
The operator got off the PC5500 Excavator to relieve themself and when operator turned to walk back to the ladder, operator ran into the lube arm that was down for the service. The operator suffered a cut on the left side of head just above the temple.
Employee was trying to flip a D11 cutting edge over and the crowbar slipped out of the cutting edge causing them to slip and fall on a strut that was laying on the ground.
Employee was operating a D10 T Dozer when EE backed over a rock and jarred self. The employee went to the ER the next day took off a couple days came back to work and is off again awaiting an MRI.
Employee picked up a piece of metal and stated EE felt something pop in their left shoulder. The Employee continued to work, and said it was ok. On 1/24/2025 EE informed the foreman that EE had scheduled a surgery for their shoulder for 1/28/2025.
Employee was working on personal vehicle in the mine parking lot. While changing the brakes, the vehicle fell off the car jack and onto EE's right lower leg. EE was transported to the hospital by ambulance and received stitches, x-ray (nothing broken) and treated with medication.
The employee was climbing back onto the Dozer, EE's foot slipped off the step next to the engine and EE's left leg fell between the track and the frame. EE's knee struck against the track causing a fracture. EE was taken to the hospital by ambulance and treated for the fracture.
Employee was rolling up the grease line when EE lost control of the hose, the handle caught EE's hand and caused a strained tendon.

Employee was operating an 830E Komatsu Rock truck. While being loaded a rock slipped out of the bucket hitting the top side rail of the truck bed jarring the Employee. The employee went to the hospital complaining of back pain and tingling in right foot.
Employee was conducting a pre-operational exam of EE's truck. EE raised the hood and felt a sharp pain in EE's back. EE continued to work for a few hours and then went to the hospital. EE was treated for a strain and given a couple days off to rest.
Employee was removing Struts on a rock truck. The left strut had been removed and EE was cutting the bottom pin out from the right Strut. The right strut moved when they cut through the pin. They stepped back and tripped over the new struts laying on the ground behind them. They landed on the second strut cracking a rib and vertebra.
Drill was snow covered and turned into ice overnight. While working on drill, the back door wouldn't open, so the technician moved to the back left corner to continue working, when standing up from back left corner, feet slipped on ice causing technician to twist left knee.

Employee was climbing upon a Rock Truck, when EE's foot hit the first platform on the lower level EE's foot slipped and EE lost grip falling on to the ground approximately 3-4 feet on EE's left side. EE was transported to the Hospital and later released that evening.
Employee was shoveling tracks on an excavator, felt pain in their lower back. Went to the doctor the next day and was told to rest a couple days.
Employee stopped EE's service truck in an area where a drill had been repaired. When EE walked around to shut a cabinet door EE stepped into a drill hole. EE received a sprain left ankle and a small fracture in EE's foot.

EE was taken by ambulance to the hospital. EE was X-rayed, no broken bones and had stitches in hand, cuts and bruises.
Employee was cutting bolts off a dozer blade when the hose connected to the torch ruptured and caused a flash fire. The employee had severe burns on EE's left arm and chest. EE was transported to Logan Hospital and later transported to Cabell Huntington burn unit for treatment.
While Employee was cleaning up rock in the pit where the rock trucks where backing in, unaware that Employee was on an unstable rock under the excavator Employee exstended the arm out to move a rock and the excavator tipped over on its right side. The operator recevied two cuts to head and cuts to right hand. Employee was wearing seat belt.
Employee was checking the antifreeze in a loader, when employee started to remove the cap the raidiator was still hot and the antifreeze scalded employee's left hand. The employee went to the doctor and was treated for burns.
Employee was operating a rock truck, leaving the pit area into the valley fill they hit a bump in the road and said their lower back was hurting. They left around 11:00 PM and went to the doctor the nest day. X-rays where done and they were treated for a strained back.

The employee was working at the new warehouse area near the Rock truck Parkup. Employee was laying a piece of metal down when employee felt a pull in lower back. Employee was taken to the Hospital and treated for a pulled muscle.
Pole was laying on side of berm. Pole was scotched. Turned auger and accidently hit pole. Pole rolled down hill and hit employee who sustained a leg injury.
Employee went out before the shift to help mechanic trouble shoot employee's grader. After helping a few minutes employee complained of chest and arm pain. The mechanic brought employee back to the portal area where first aid was given until paramedics arrived.
Employee was hauling across the mine road, the derate indicator light came on so EE stopped the truck. The stop engine light came, smoke and flames began to rise from the drivers side hood. The flames had gotten to intense before EE could raise the hood so EE backed away until help arrived. The trailer was saved but the Tractor was a total loss. No injuries reported.
Employee was attempting to walk through mud when foot got stuck and lost balance. Employee fell and put out right arm to catch self, employee continued to work the rest of the shift and worked the following day. Employee went to the doctor, the result was a compression fracture in the right arm.
Employee was in the West Pit area of the mine, when EE tripped over frozen mud. EE tried to catch self and hit hand on the ground, EE finished the shift and went to the hospital that night, the result was a fractured finger.
Employee was exiting an excavator, EE placed right foot onto the step that had dirt and mud on it. EE's foot slipped striking knee against the step causing EE's knee to swell.
The employee was dismounting a Hydro seed truck when employee jumped from the rear bumper landing awkwardly straining right ankle.
Employee was replacing a starter on a Rock Truck. The employee stated it was a hard area to access. When EE lifted the starter, EE felt something in their back. The employee did not report until 11/28/2022 and did not go to the Doctor until 12/5/2022. EE has a pinched nerve in their back.
