Cleaning coal bins out going in back to start the belts, was stepping over a frozen coal pile, between processing plant and a concrete wall. Stepped over with right foot, slipped on ice and was thrown into the concrete wall with left shoulder, then fell onto buttocks, hurting shoulder and back. We had 40 mile per hour wind this day.

Moving a wooden block to secure a piece of equipment and EE stepped into a rut causing EE to drop block of wood on left foot. Per phone call the foot was broken.
