Employee was climbing onto a Cat 777B End Dump when he heard a pop in his right knee. The accident happened on 12/13/2013 but the employee continued to work until 04/07/2014.
Employee was operating an end dump on the spoil dump when he went to dump the rock load the end dump traveled off the dump backwards overturning. The employee was taken to the hospital where he received 21 stitches total to the right forearm and left check area.

Hydraulic hose end blew off causing a flash fire. Employee received slight burns to arms. Treated and released for the burns. While leaving truck, employee struck his right knee and bruised his heel. Company doctor took employee off work until knee and foot healed.

Employee was cleaning in the prep plant with a shovel. When he twisted around he felt pain in his knee. Employee worked until 08/15/2009. It took until 09/16/2009 for it to be determined from a peer review if this was a new injury and not a re injury. This is why the 10 day limit was passed.

Driver was on top of his loaded trailer, moving loose coal. He fell off of the trailer, estimated height 12 feet. He landed on his back. Driver was not using PPF.

The Employee slipped and fell on the ice. The Employee has been treating since that time thinking it would heal with exercise and therapy. In the end they had to go in and repair a small tear causing him to become a lost time on 08/29/2008. Employee is expected to be back on restricted duty on 09/08/2008.

The clay bank in pit #1 slid into the coal pit, shoving an end dump truck which slid into the loader. Five inches of rain, 36 hours before the slide, was a contributing factor.

EE was using a grinder when it kicked back off of the metal. The grinder slipped from his grip and struck his neck. He went to the doctor after work and received two stitches.
EE climbing onto his coal loader in the coal yard. He slipped climbing up the loader ladder and caught himself with his right arm causing some damage in his shoulder. This has been a medical only until 01/24/2007 when it went lost time.

Ground was frozen in the morning and as the sun hit the haul road in this location it became slick. Truck became sidways on road and when it contaced a dry area it tipped over on its side.

Employee had just put a pin in the ripper and as he was standing up the ripper shifted causing the chain from the boom to drop hitting him on the shoulder.
EE was climbing onto dozer. He slipped and fell between push arms & track frame, causing pain to left knee & lower back. (HAD ARTHROSCOPIC KNEE SURGERY ON 8/18/05.)
