Employee was climbing onto a Cat 777B End Dump when he heard a pop in his right knee. The accident happened on 12/13/2013 but the employee continued to work until 04/07/2014.

Employee was dumping wet material on the upper west dump when the dump gave way. The end dump traveled backwards over the dump before over turning.

Employee was operating an end dump on the spoil dump when he went to dump the rock load the end dump traveled off the dump backwards overturning. The employee was taken to the hospital where he received 21 stitches total to the right forearm and left check area.

Employee was lifting crusher hammers and felt a strain in his right shoulder.

A chain broke, causing a piece of metal to fall and strike EE's hand.

Employee was moving a piece of pipe with a chain fall, the load became unbalanced and tilted and struck employee's right foot. Causing a small fracture.

Inspecting transmission gears, gears rolled and pinched finger.

Employee was struck by a chain he was installing.

EXITING SCALES TRACTOR TRAILER UNIT WAS DRIVEN ON SHOULDER AND ROLLED OVER ONTO ITS SIDE INJURING DRIVER INSIDE.

Hydraulic hose end blew off causing a flash fire. Employee received slight burns to arms. Treated and released for the burns. While leaving truck, employee struck his right knee and bruised his heel. Company doctor took employee off work until knee and foot healed.

Employee was cleaning in the prep plant with a shovel. When he twisted around he felt pain in his knee. Employee worked until 08/15/2009. It took until 09/16/2009 for it to be determined from a peer review if this was a new injury and not a re injury. This is why the 10 day limit was passed.

Employee was unhooking a pump hose when the end came loose and struck his cheek.

Employee's seat bottomed out and hurt his back.

Fuel hose struck his nose. On medication and cannot work.

Driver was on top of his loaded trailer, moving loose coal. He fell off of the trailer, estimated height 12 feet. He landed on his back. Driver was not using PPF.

Repairing on dryer.

Employee was getting a chain out of a tool box, when the lid of the box fell on his finger.

Employee was cutting a piece of belting, when his knife slipped and cut his arm, requiring a few stitches.

Employee strained his right shoulder climbing off a piece of equipment; he has been working and going to therapy but the doctor decided to take him off work for further treatment on 09-29-2008.

Employee slipped and stepped into a hole and twisted his leg.

The Employee slipped and fell on the ice. The Employee has been treating since that time thinking it would heal with exercise and therapy. In the end they had to go in and repair a small tear causing him to become a lost time on 08/29/2008. Employee is expected to be back on restricted duty on 09/08/2008.

Employee was cleaning ice on refuge belt roller, when glove caught on roller.

Repairing in the wash plant changing an 8 in pipe when a piece broke and fell striking his lower leg.

Employee had a socket break in his hand cutting his palm requiring stitches.

The clay bank in pit #1 slid into the coal pit, shoving an end dump truck which slid into the loader. Five inches of rain, 36 hours before the slide, was a contributing factor.

Employee was operating a dozer when he backed over a rock and strained his neck and shoulder.

EE was using a grinder when it kicked back off of the metal. The grinder slipped from his grip and struck his neck. He went to the doctor after work and received two stitches.

Employee was working on an D-11 dozer putting the track together when his chain slipped causing the track to strike his knee.

EE was performing repair work on the stacker belt, when he fell off stacker belt and bruised his ribs.

EE climbing onto his coal loader in the coal yard. He slipped climbing up the loader ladder and caught himself with his right arm causing some damage in his shoulder. This has been a medical only until 01/24/2007 when it went lost time.

HE WAS HELPING THE MECHANIC PUT A GUARD ON THE RIPPER SHANK OF A D-11 DOZER, HE WAS HOLDING THE KEEPER FOR HIM. THE MECHANIC SLIPPED MASHING/CUTTING EE'S MIDDLE RIGHT FINGER.

Cut left small finger with a broken hose clamp.

LIFTING LIGHT PLANT ONTO HITCH OF TRUCK. LIGHT PLANT SLIPPED THROUGH HIS HAND AND FELL. IT LANDED ON HIS FOOT.

Employee climbed tank trailer and fell.

Slipped while climbing onto Bobcat Loader. Strained back muscle. Was medical only at the time but turned into lost time accident when employee had surgery on 4/26/06.

Still dark and snowing and visibility was poor. Dozer was pushing dirt up on stockpile when truck slightly struck tractor cab.

Changing spirals. Pushing up on the "t" to pull hose down onto it.

Changing dryer screen at wash plant. Screen was worn on and sharp on edges. Victim sliced his little finger on side of screen.

Ground was frozen in the morning and as the sun hit the haul road in this location it became slick. Truck became sidways on road and when it contaced a dry area it tipped over on its side.

Employee had been fueling equipment and he drove to the 988 loader. When he was getting out of the truck his foot slipped and he fell out onto the pile of coal. He had fuel on the bottom of his boot making it slick.

Cable on mechanics truck broke letting belly pan fall. Struck right hand fracturing little finger.

Air arcing on a wash plant screen and received a flash burn on both eyes.

HE WAS PUSHING A BAR INTO THE SCREEN AND BENT HIS WRIST BACK.

Employee had just put a pin in the ripper and as he was standing up the ripper shifted causing the chain from the boom to drop hitting him on the shoulder.

A 992 LOADER WAS LOADING THE TRUCK WITH DIRT. SAID THE JARRING AROUND HURT HIM

PULLING HOSES OVER SPOIL AND FELL HURTING HIS SHOULDER AND NECK

Working on Grease Truck, turned and hit forhead on ladder requiring stitches.

Treating pond. Opened a container of caustic soda when wind picked up and blew pellet onto eyebrow. Melted by sweat and ran into right eye.

EE was climbing onto dozer. He slipped and fell between push arms & track frame, causing pain to left knee & lower back. (HAD ARTHROSCOPIC KNEE SURGERY ON 8/18/05.)

Loading hydraulic pump into back of parts truck. Pump flipped over pinning finger between pump and truck bed.
