Counterweight on shovel was being removed. Three of the bolts holding the weight were off, the fourth bolt was loose. Employee placed left middle finger under washers and the counterweight was slightly lifted pinching fingertip. Pinched off 1/8 inch off end of fingertip.

REMOVING PUMP FROM A FLOAT, EMPLOYEE HAD RIGHT HAND ADJACENT TO CHAIN, WHEN CHAIN SLIPPED, CATCHING THE RIGHT INDEX FINGER IN PINCH POINT BETWEEN THE CHAIN & THE PUMP AND TEARING THE RIGHT IND EX FINGER OFF AT FIRST JOINT PAST THE KNUCKLE.
